À propos de Chirurgie de l’épilepsie

Careful assessment comes first, usually involving video-EEG monitoring, MRI and sometimes PET or invasive electrodes to locate the seizure focus. If a clear target exists, the surgeon removes it, as in temporal lobectomy, or disconnects it. Newer options include laser ablation and neurostimulation devices for those not suited to resection.

The chance of becoming seizure-free depends strongly on the type of epilepsy and whether the focus is clearly defined on tests. Temporal lobe epilepsy with a visible scar on MRI does particularly well. Some people can reduce medicines over time, but this is decided slowly and with specialist guidance. Risks depend on the area of brain involved.

The assessment phase can itself take days to weeks, followed by around ten days in hospital for surgery. Bring EEG records, MRI images and a seizure diary. Continue medicines as instructed, and plan epilepsy follow-up at home, as medicine changes and driving advice are decided in the months afterward.

Généralement adapté si

  • Drug-resistant focal epilepsy, usually after two or more suitable medicines have failed.
  • A clearly identified seizure focus that can be removed safely.
  • A lesion on MRI, such as hippocampal sclerosis or a tumour, matching EEG findings.
  • Patients willing to take part in a full pre-surgical evaluation.

Il peut falloir attendre si

  • Seizures arising from multiple unlinked areas or without a clear focus.
  • Focus in an area essential for language or movement that cannot be spared.
  • Severe medical illness or active infection.
  • Seizures not truly epileptic, such as functional seizures.
Comment se préparer
  1. Bring all EEG reports, MRI and PET images, and a full list of anti-epileptic medicines tried.
  2. Do not change your anti-epileptic medicines unless your neurologist instructs, as the team will plan how they are handled.
  3. Expect video-EEG monitoring, neuropsychological testing and sometimes Wada or functional MRI testing.
  4. Keep a seizure diary and video of typical events.
  5. Plan a longer stay, since the full evaluation may take days or weeks.
Questions à poser à votre chirurgien
  1. Which type of epilepsy do I have, and where is the focus?
  2. Which operation do you propose, and what is the chance of seizure freedom in my situation?
  3. What risks apply to memory, speech and vision?
  4. How many epilepsy operations does your centre do each year?
  5. When can I reduce medicines, and who decides?
  6. What alternatives, such as RNS, VNS or LITT, were considered?
Signaux d'alerte à surveiller en comparant les hôpitaux
  • No comprehensive epilepsy centre with video-EEG and neuropsychology.
  • Surgery offered without locating the seizure focus.
  • Guarantees of seizure freedom.
  • No plan for medicine management after surgery.
