Velopharyngeal Insufficiency
Velopharyngeal insufficiency means the soft palate and throat muscles do not close properly during speech, letting air escape through the nose. It can follow cleft palate repair or occur with a short or poorly moving palate.

À propos de velopharyngeal insufficiency
Ce qu'il faut savoir
Speech sounds nasal, consonants may be weak and some children develop compensatory habits. Assessment is by a speech and language therapist, nasendoscopy and sometimes videofluoroscopy to see how the palate moves. Without treatment, the speech problem usually persists, and early assessment improves the chance of good results.
Speech therapy is tried first, particularly when closure is only slightly incomplete. A palatal lift appliance is an option for some people. When a clear gap persists, surgery such as pharyngeal flap, sphincter pharyngoplasty or posterior wall augmentation is performed. Teams specialising in cleft and speech surgery are sometimes sought abroad.
Bilan avant le départ
Examens à déjà réalisés
- A speech and language therapist's assessment report describing nasal resonance, nasal air escape and articulation.
- Nasendoscopy or videofluoroscopy reports with images or video, which show the pattern of palate and throat movement.
- Your cleft or palate surgery history, including operation notes and dates, if applicable.
- Hearing test results and any history of ear infections, as these often accompany palate conditions.
- A recording of your or your child's speech, and a sleep history, because some operations can affect breathing during sleep.
Deuxième avis
Quand obtenir un second regard
- Surgery is advised but no instrumental assessment such as nasendoscopy has been done.
- You have been advised to have a procedure meant for snoring or sleep apnoea, which does not usually treat this speech problem.
- Speech has not improved after a previous palate or pharyngeal operation.
- The cause is unclear, for example when the child has no cleft, and a hidden palate abnormality or a syndrome needs checking.

Questions à poser
- What is the cause of the air escape and how severe is it?
- Could speech therapy alone improve this, and for how long should we try it?
- Which operation suits my gap pattern, such as a pharyngeal flap or sphincter pharyngoplasty?
- What are the risks to breathing and sleep after surgery?
- How soon after surgery will speech improve, and will therapy still be needed?
- What happens if the gap remains after surgery?
Perspectives
Pronostic réaliste
Many people with velopharyngeal insufficiency improve substantially after properly chosen surgery combined with speech therapy, and a majority achieve acceptable speech. The best operation depends on the size and shape of the gap seen on assessment. Improvement may take months as speech habits are retrained. A minority need further surgery. Possible effects include snoring or sleep-disordered breathing after surgery, which need monitoring.

Bilan diagnostique

04 · FAQ
Questions fréquentes sur velopharyngeal insufficiency
Can speech therapy fix it?
It may help mild cases or articulation habits, but a structural gap usually needs surgery. Therapy is still needed afterwards.
How is VPI diagnosed?
By a speech therapist's assessment plus nasendoscopy or videofluoroscopy, which show how the palate and throat close during speech.
Will surgery affect breathing?
The operation narrows the airway behind the palate, so snoring or sleep breathing problems can occur and are monitored.
How long until speech improves?
Improvement often starts within weeks, but retraining speech with therapy can take months.
Clinical context
Working up velopharyngeal insufficiency properly
A condition like velopharyngeal insufficiency is rarely well-served by jumping straight to treatment shopping. The sequence below is what a orl / oto-rhino-laryngologie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or orl / oto-rhino-laryngologie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
