À propos de bêta-thalassémie majeure
Ce qu'il faut savoir
Babies become pale, tired and poorly growing in the first year, with an enlarged spleen and bone changes. Blood tests and haemoglobin analysis confirm the diagnosis, and genetic testing identifies the mutations. Without treatment, severe anaemia and heart failure develop in childhood.
Standard care is regular blood transfusions every few weeks, with iron chelation tablets or infusions to remove the excess iron they cause. This keeps most children well but needs lifelong commitment. A matched-donor bone marrow (stem cell) transplant is the only established cure and gives the best results in young children with a matched sibling. Families often travel abroad for transplant when a donor and specialist unit are not available locally.
Bilan avant le départ
Examens à déjà réalisés
- Haemoglobin electrophoresis or HPLC and genetic (mutation) reports confirming the type of thalassaemia, for the patient and ideally both parents.
- A transfusion record showing frequency, volume and any reactions, plus the red-cell antibody screen.
- Serum ferritin trend, and liver and cardiac MRI T2* results showing iron overload.
- Details of iron chelation drugs used, doses, side effects and adherence.
- HLA typing of the patient and siblings, and results for hepatitis B, hepatitis C and HIV.
Deuxième avis
Quand obtenir un second regard
- A transplant has not been discussed, even though a healthy matched sibling donor may exist.
- Iron overload remains high despite chelation, or heart or liver MRI shows iron deposition.
- A haploidentical or unrelated donor transplant is proposed, and you want to compare the risks with lifelong transfusions.
- The child has frequent transfusion reactions or has developed red-cell antibodies.

Questions à poser
- Is a stem cell transplant right for my child, and is there a suitable donor?
- What are the chances of cure, and the risks of graft failure and graft-versus-host disease?
- How does age and the amount of iron overload affect transplant results?
- How is iron overload being monitored, and is the current chelation enough?
- What would life look like with ongoing transfusions compared with after transplant?
- Will the family need genetic counselling for future pregnancies?
Perspectives
Pronostic réaliste
Without transplant, thalassaemia major is managed by regular transfusions and iron chelation, and people who stick to treatment can live into adulthood, though iron overload damages the heart, liver and endocrine glands if poorly controlled. A matched-sibling stem cell transplant, especially in young children before organ damage, cures the large majority. Outcomes with other donors are improving but carry higher risk. Gene therapy is emerging for selected patients.

Bilan diagnostique

04 · FAQ
Questions fréquentes sur bêta-thalassémie majeure
Can thalassaemia major be cured?
Yes. A stem cell transplant can cure it, with the best results using a matched sibling donor in young children before organ damage. Gene therapy is emerging for selected patients.
Why is iron chelation needed?
Regular transfusions deposit iron in the heart, liver and glands, and the body cannot remove it. Chelation medicines prevent this damage, and adherence is essential.
Can thalassaemia be prevented in future children?
Yes. Carrier testing of parents and genetic counselling allow options such as prenatal testing or embryo testing before pregnancy.
Clinical context
Working up bêta-thalassémie majeure properly
A condition like bêta-thalassémie majeure is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or oncologie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
