Teratozoospermia

Teratozoospermia is a semen finding in which a high proportion of sperm have an abnormal shape. It may reduce natural fertility, though many men with it still father children.

À propos de teratozoospermia

Ce qu'il faut savoir

There are usually no symptoms; it is found on a semen analysis assessed against laboratory criteria. The result can vary between samples, so a repeat test after a few months is advised. Causes include infection, varicocele, smoking, heat, some medicines and genetic factors, and often none is found. Other parameters, such as count and movement, matter as well.

Lifestyle changes help some men: stopping smoking, reducing alcohol, losing weight and avoiding heat. A varicocele or infection is treated if present. Where pregnancy does not follow, intrauterine insemination may be tried, but intracytoplasmic sperm injection (ICSI) within IVF is the usual route for marked abnormalities, and IMSI uses higher magnification to select sperm. Couples sometimes travel abroad for assisted conception.

Bilan avant le départ

Examens à déjà réalisés

  • At least two semen analyses, a few weeks apart, with a laboratory report including count, motility and strict morphology.
  • Hormone tests, such as FSH, LH and testosterone, if advised by your doctor.
  • Scrotal ultrasound, to look for a varicocele or other testicular problem.
  • Female partner's fertility assessment, including ovarian reserve, tubes and ovulation.
  • A record of fevers, medicines, smoking, alcohol, heat exposure and previous treatments.

Deuxième avis

Quand obtenir un second regard

  • IVF or ICSI is being proposed on a single semen result without repeat testing.
  • A varicocele has been found, and you want to know whether treating it could improve sperm quality.
  • Previous ICSI cycles failed and no sperm DNA fragmentation or genetic test has been discussed.
  • Your morphology is very low (such as globozoospermia), where specific genetic advice is needed.

Questions à poser

  1. Is the abnormal shape the only problem, or are count and movement also affected?
  2. Is there a treatable cause, such as a varicocele, infection or lifestyle factor?
  3. Would sperm DNA fragmentation testing or genetic testing help me?
  4. When are IUI, IVF or ICSI appropriate in our situation?
  5. How common is it for the sperm abnormality to affect the baby?
  6. What can I do over the next three months to improve sperm quality?

Perspectives

Pronostic réaliste

Teratozoospermia by itself does not always prevent pregnancy, and many couples conceive naturally if other parameters are good. Sperm production takes around three months, so lifestyle changes and treating a varicocele may help over that time. When other measures fail, ICSI selects a single sperm for injection, and fertilisation rates are generally good, although success depends heavily on the female partner's age and egg quality.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur teratozoospermia

Can teratozoospermia be reversed?

Sometimes. Sperm quality can improve over about three months by stopping smoking, reducing alcohol and heat exposure, losing weight, and treating a varicocele or infection.

Does abnormal sperm shape cause birth defects?

Abnormal shape on its own does not usually mean the baby will be affected. Rare, severe forms may have a genetic cause, for which genetic counselling is advised.

Will I need ICSI?

Not always. If other semen parameters are reasonable and the female partner is fertile, natural conception or IUI may still be possible. ICSI is suggested when morphology is very low or other treatments fail.

Clinical context

Working up teratozoospermia properly

A condition like teratozoospermia is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilité et fiv specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilité et fiv specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
