Sportsman's Hernia

Sportsman's hernia, also called athletic pubalgia, is a strain or weakness of the soft tissues in the lower abdominal wall or groin. It causes lasting groin pain in athletes, usually with no visible bulge.

À propos de sportsman's hernia

Ce qu'il faut savoir

Pain is felt in the lower abdomen or groin during sprinting, twisting or kicking and may spread to the inner thigh. It often eases with rest and returns on resuming sport. Diagnosis is mainly clinical, since examination can be unremarkable. MRI helps to exclude hip problems, true hernias and bone stress changes. It is not dangerous, but can end a sporting season if ignored.

Rest and a structured rehabilitation programme, with core and hip strengthening, resolve many cases over several months. Injections may be tried for persistent pain. If proper rehabilitation fails, surgery reinforces the weakened abdominal wall, either by keyhole repair with mesh or by open tissue repair, and most athletes return to sport in a few weeks. Treatment abroad is sometimes sought from surgeons with specific experience in athletes.

Bilan avant le départ

Examens à déjà réalisés

  • MRI of the pelvis and groin, ideally with a sports-specific protocol, to look for pubic bone stress, tendon injury and other causes.
  • Ultrasound of the groin, ideally performed dynamically, to exclude a true inguinal hernia.
  • A record of the rehabilitation programme you have followed, including its length and how your pain responded.
  • Details of your sport, training load and which movements, such as sprinting or kicking, provoke the pain.
  • Any hip X-rays or MRI, because hip problems commonly cause similar groin pain.

Deuxième avis

Quand obtenir un second regard

  • Groin pain has continued for months despite structured physiotherapy, and the diagnosis has never been confirmed by a groin specialist.
  • Surgery has been proposed but the hip joint has not been properly assessed as a cause.
  • Different clinicians have given different diagnoses, such as adductor tendinopathy, hip impingement or hernia.
  • You are an athlete and need a clear return-to-play plan before committing to an operation.

Questions à poser

  1. Is my pain coming from the abdominal wall, the adductor tendons, the pubic bone or the hip?
  2. Has a true inguinal hernia been ruled out?
  3. How long should structured rehabilitation continue before surgery makes sense?
  4. What does the operation involve, and does the type of repair change recovery?
  5. When can I realistically return to training and competition?
  6. What are the chances the pain will come back, and how can I reduce that risk?

Perspectives

Pronostic réaliste

Sportsman's hernia is not life-threatening, and many athletes improve with a well-structured rehabilitation programme over several months. When pain persists and the diagnosis is secure, surgery or repair allows most athletes to return to their sport, often within a few months. Results are less good when another source, such as the hip, has been missed, so accurate diagnosis matters most.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur sportsman's hernia

Is a sportsman's hernia a true hernia?

Usually not. There is often no visible bulge. It describes strain or weakness in the soft tissues of the lower abdominal wall and groin, and a proper assessment also looks for hip and adductor causes.

Will I need surgery?

Many athletes improve with several months of specific rehabilitation. Surgery is considered when a confirmed diagnosis does not improve after a proper programme, particularly if you want to return to a demanding sport.

When can I return to sport after treatment?

After rehabilitation, return is staged as pain settles. After surgery, many athletes return to full training in roughly two to three months, but timing depends on the operation and your sport, and your surgeon will set milestones.

Clinical context

Working up sportsman's hernia properly

A condition like sportsman's hernia is rarely well-served by jumping straight to treatment shopping. The sequence below is what a chirurgie digestive specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or chirurgie digestive specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
