Spondylolisthésis

Spondylolisthesis is when one vertebra slips forward over the one beneath it, most often in the lower back. It results from a defect in the bone, often in young athletes, or from age-related wear.

À propos de spondylolisthésis

Ce qu'il faut savoir

Typical symptoms are lower back pain, tight hamstrings and, if a nerve is squeezed, leg pain, numbness or weakness when standing or walking. It is diagnosed with standing X-rays, which show the slip, and MRI to look at the nerves. Many people stay stable for years, and slips seldom progress much in adults.

Treatment starts with activity changes, physiotherapy to strengthen the core, anti-inflammatory medicines and sometimes spinal injections. Surgery is considered if pain or nerve symptoms persist for several months or the slip is large or progressing. Decompression frees the nerves, and fusion with screws and rods stabilises the slipped segment. People sometimes seek treatment abroad for fusion when waits are long or minimally invasive techniques are wanted.

Bilan avant le départ

Examens à déjà réalisés

  • Standing lumbar X-rays including flexion and extension views, which show whether the slip moves and how far it has progressed.
  • Lumbar MRI, with images on disc, to show the degree of nerve compression.
  • A written account of leg pain, numbness or weakness, walking distance and any bladder or bowel changes.
  • Details of physiotherapy, injections and pain medicines already tried and how long each was tried for.
  • Your general fitness details, such as bone density results if available, smoking status and diabetes control.

Deuxième avis

Quand obtenir un second regard

  • Surgery has been advised but you have not had a proper trial of physiotherapy and activity changes.
  • You have worsening leg weakness, foot drop or bladder changes, which need prompt specialist review.
  • Fusion is recommended and you want to know whether decompression alone would be enough.
  • The slip has been described differently in different reports, such as the grade changing between scans.

Questions à poser

  1. What grade is my slip, is it stable, and is it likely to progress?
  2. Is the main problem my back, or nerve pressure causing leg symptoms?
  3. Would decompression alone work for me, or would a fusion be needed?
  4. What non-surgical treatments are still worth trying, and for how long?
  5. What are the realistic recovery milestones for returning to work and lifting?
  6. What happens to the levels above and below a fusion over the years?

Perspectives

Pronostic réaliste

Many people with spondylolisthesis improve with exercise, activity changes and sometimes injections, and low-grade slips rarely progress in adults. Surgery is considered when leg pain, weakness or disabling symptoms persist. When fusion or decompression is done for the right reasons, most patients report clear relief of leg pain, with back pain relieved less predictably. Outcomes depend on slip grade, age, smoking and general health.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur spondylolisthésis

Do I always need surgery for spondylolisthesis?

No. Most people improve with exercise, physiotherapy, activity changes and sometimes injections. Surgery is usually reserved for persistent leg pain or weakness, a progressing slip, or symptoms that limit daily life despite months of non-surgical care.

Will the slip keep getting worse?

In adults, most low-grade slips change little over time. Progression is more likely in young people with a high-grade slip or in a few adults with an unstable segment, so follow-up X-rays are often advised.

Can I fly or travel with spondylolisthesis?

Usually yes. Long flights can aggravate back pain, so choose an aisle seat, stand and walk regularly, and keep your medicines in hand luggage. After spinal surgery, your surgeon will advise when it is safe to fly.

Clinical context

Working up spondylolisthésis properly

A condition like spondylolisthésis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a orthopédie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or orthopédie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
