À propos de retinopathy of prematurity (adult sequelae)
Ce qu'il faut savoir
Many people are short-sighted and have a dragged retina or macula, reduced sight or a squint, and are prone to retinal tears, detachment, glaucoma and cataract. Floaters, flashes or a shadow in the vision need urgent examination. Diagnosis is by dilated examination, wide-field photographs, optical coherence tomography and ultrasound if the view is poor.
Many adults need only regular retinal checks and glasses. Laser can seal weak peripheral retina and tears. Retinal detachment and traction are treated surgically, usually with vitrectomy through the pars plana, sometimes with gas, oil or a scleral buckle. Outcomes depend on the macula's condition. Patients may travel for a vitreoretinal surgeon experienced in complex paediatric-onset cases.
Bilan avant le départ
Examens à déjà réalisés
- Dilated retinal examination reports from a retina specialist, describing any traction, retinal folds, tears or detachment in each eye.
- OCT scans of the macula and wide-field fundus images, ideally with comparison to earlier ones.
- Visual acuity, refraction and eye pressure measurements, noting high myopia and any history of glaucoma.
- Records of your birth history and any treatment for retinopathy of prematurity in infancy, such as laser or injections.
- Details of previous eye surgery, cataract or retinal detachment in either eye.
Deuxième avis
Quand obtenir un second regard
- You have new flashes, floaters or a shadow in your vision. This is an emergency, so seek urgent review.
- You have been advised to have vitrectomy but your vision is stable and the benefit is unclear.
- Only one eye sees well, so any operation puts your main vision at risk.
- The retina has already detached once, or you have glaucoma or cataract developing alongside retinal problems.

Questions à poser
- Which retinal changes do I have and how likely are they to progress?
- Is surgery needed now, or is observation with regular examinations safer?
- What are the chances vitrectomy keeps the retina attached, and what visual result is realistic?
- What risks exist for cataract, glaucoma and re-detachment after surgery?
- How often should I be examined, and which symptoms need immediate attention?
- Do I need glasses, contact lenses or low-vision support, and are my children at risk?
Perspectives
Pronostic réaliste
Many adults with old retinopathy of prematurity changes remain stable for years, while others develop tears or detachments that need surgery. Operations on scarred retinas are technically difficult and the result depends on how severe the traction is. Surgery usually aims to stabilise vision rather than restore normal sight, and lifelong eye checks are recommended.

Bilan diagnostique

Questions fréquentes sur retinopathy of prematurity (adult sequelae)
Can retinopathy of prematurity get worse in adulthood?
The active disease settles in infancy, but scarring can cause late problems such as retinal tears, detachment, glaucoma and cataract. Regular lifelong eye examinations are advised.
Is surgery always needed?
No. Many adults only need monitoring. Surgery is considered for detachment, tears or traction threatening the macula.
Will surgery restore my sight?
Surgery aims to keep the retina attached and prevent further loss. Normal vision is often not regained because the retina may be permanently scarred or underdeveloped.
Clinical context
Working up retinopathy of prematurity (adult sequelae) properly
A condition like retinopathy of prematurity (adult sequelae) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ophtalmologie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or ophtalmologie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
