À propos de pulpal necrosis
Ce qu'il faut savoir
Sometimes it causes a toothache, tenderness on biting or a darkened tooth, but it can be painless until an abscess forms with swelling, a gum boil or a bad taste. Diagnosis involves cold and electric pulp tests, tapping, and an X-ray that may show a dark area at the root tip. Untreated, infection can spread to the jaw and face.
Root canal treatment cleans out the dead tissue, disinfects the canals and seals them, followed by a crown or filling to protect the tooth. If an earlier root filling has failed, endodontic retreatment is tried, and a small root-end surgery (apicoectomy) may be used for persistent infection. Extraction, with an implant or bridge, is the alternative when the tooth cannot be saved. Treatment abroad is mainly considered when a specialist endodontist is needed.
Bilan avant le départ
Examens à déjà réalisés
- Recent dental X-rays (periapical, or a small-volume CBCT) showing the root canals and any infection at the root tip.
- Dentist's notes on pain, swelling, sensitivity tests and how the tooth responds to cold and tapping.
- Records of previous root fillings or crowns on the tooth, with dates.
- A list of antibiotics taken and any allergies, plus medical history such as diabetes or heart conditions.
- Photographs of any swelling or draining sinus (gum pimple).
Deuxième avis
Quand obtenir un second regard
- Extraction is advised for a tooth that may be restorable with root canal treatment.
- Pain or infection persists after a root canal and retreatment has not been discussed.
- A crown or post is needed and you are unsure the tooth has enough structure left.
- Facial swelling, fever or difficulty swallowing, which should not wait for a routine opinion.

Questions à poser
- Is the tooth saveable, and what proportion of healthy structure is left?
- How many visits are needed, and will a specialist endodontist treat it?
- What is the chance of success, and what could cause failure?
- Will the tooth need a crown afterwards, and when?
- What happens if the infection is left untreated?
- When would retreatment, surgery at the root tip or extraction be better?
Perspectives
Pronostic réaliste
Root canal treatment saves most teeth with pulpal necrosis, with success rates commonly reported around 85 to 95 percent in well-done treatment. Outcomes depend on how well the canals are cleaned and sealed, the extent of infection at the root tip and the quality of the final restoration. Teeth can be retreated, and a crown usually protects the tooth long term.

Bilan diagnostique

04 · FAQ
Questions fréquentes sur pulpal necrosis
Can a dead tooth be saved?
Often yes. Root canal treatment removes the infected tissue and seals the tooth, which can then function for many years, usually with a crown.
Will it hurt?
The procedure is done under local anaesthetic and is usually comfortable. Some soreness for a few days after is common.
What if I do nothing?
Infection can spread to the jaw and face, cause abscess and bone loss and eventually lead to loss of the tooth. It can become serious.
Is an extraction better?
Extraction is quicker but loses the tooth, and replacing it takes more time. Saving the tooth is usually preferred if possible.
Clinical context
Working up pulpal necrosis properly
A condition like pulpal necrosis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a dentaire specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or dentaire specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
