Pseudoexfoliation Glaucoma

Pseudoexfoliation glaucoma is an open-angle glaucoma in which flaky protein material blocks the eye's drainage and raises pressure. It usually affects people over 60.

À propos de pseudoexfoliation glaucoma

Ce qu'il faut savoir

There are often no early symptoms, and vision is lost gradually from the edges. Pressure tends to be higher and swing more than in other types of glaucoma. Examination of the pupil edge may show flaky white deposits, and diagnosis includes pressure measurement, optic nerve imaging and visual field tests. The weakened lens support also makes cataract surgery more complex.

Treatment starts with pressure-lowering eye drops, and laser trabeculoplasty tends to work well in this type. If pressure remains high or the optic nerve keeps deteriorating, surgery is considered: trabeculectomy for lower target pressures or minimally invasive glaucoma surgery (MIGS) for milder disease, often combined with cataract removal. Patients who need surgery sometimes seek a specialist glaucoma surgeon abroad.

Bilan avant le départ

Examens à déjà réalisés

  • Eye pressure readings over time, including peaks and any pressure spikes.
  • Visual field tests (perimetry), repeated over time, showing any progression.
  • OCT scans of the optic nerve and nerve fibre layer.
  • Slit-lamp and gonioscopy findings, including flaky material on the lens, pupil margin or drainage angle.
  • Cataract status, since cataract surgery in pseudoexfoliation needs special planning for weak lens support.

Deuxième avis

Quand obtenir un second regard

  • Your eye pressure fluctuates and your drops are not keeping it under control.
  • Cataract surgery is planned and nobody has mentioned the weak lens support seen in pseudoexfoliation.
  • Your visual field is worsening despite 'normal' clinic pressure readings.
  • Surgery has been proposed and you want to compare trabeculectomy with newer minimally invasive procedures.

Questions à poser

  1. What is my target pressure, and is my current treatment reaching it?
  2. Is my lens loose, and does that affect cataract surgery planning?
  3. Would laser treatment or minimally invasive surgery help, or do I need a trabeculectomy?
  4. How quickly is my damage progressing based on my fields and scans?
  5. What are the risks of trabeculectomy, such as low pressure or infection?
  6. Should family members be checked?

Perspectives

Pronostic réaliste

Pseudoexfoliation glaucoma tends to have higher pressures and faster progression than ordinary open-angle glaucoma, but sight can usually be preserved with timely treatment and close follow-up. Surgery is more often needed. The outcome depends on the stage at diagnosis, how well pressure is controlled and regular monitoring of the optic nerve and visual field.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur pseudoexfoliation glaucoma

Is pseudoexfoliation glaucoma worse than normal glaucoma?

It is often more aggressive, with higher pressure swings, so it needs closer follow-up and earlier surgical treatment.

Can I still have cataract surgery?

Yes, but the surgeon needs to know in advance, as the lens supports can be weak. Specialised techniques and devices reduce risks.

Will I need surgery?

Many patients do eventually. Drops and laser are usually tried first, and the decision depends on how well pressure is controlled.

Will I lose my sight?

Most people keep useful vision with treatment and regular monitoring. Lost vision cannot be restored, so early control matters.

Clinical context

Working up pseudoexfoliation glaucoma properly

A condition like pseudoexfoliation glaucoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ophtalmologie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ophtalmologie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
