À propos de insuffisance ovarienne prématurée

Ce qu'il faut savoir

Women may notice missed or irregular periods, hot flushes, vaginal dryness, mood changes and difficulty conceiving. Diagnosis is confirmed by raised follicle-stimulating hormone on two blood tests at least a month apart, with low oestrogen, and other causes such as pregnancy, thyroid disease and high prolactin are excluded. Chromosome and autoimmune tests may be done. Occasional ovarian activity can return, but natural pregnancy is uncommon.

Hormone replacement therapy is recommended until around the natural age of menopause, to protect bones, heart and wellbeing. Calcium, vitamin D and exercise support bone health. For women wanting a pregnancy, IVF with donor eggs is the most reliable option, and some women explore it abroad where donor programmes are more available. Emotional support and counselling are important.

Bilan avant le départ

Examens à déjà réalisés

  • Blood tests showing raised FSH on two occasions at least a month apart, with oestradiol and AMH results.
  • Karyotype and FMR1 (fragile X) results, and thyroid and adrenal antibody tests, which look for causes.
  • Pelvic ultrasound report with antral follicle count and uterine lining measurement.
  • Bone density (DEXA) scan, since low oestrogen weakens bones early.
  • History of chemotherapy, pelvic radiotherapy, ovarian surgery or autoimmune disease, plus any hormone therapy already taken.

Deuxième avis

Quand obtenir un second regard

  • Your diagnosis rests on a single blood test, as FSH can fluctuate and some women still ovulate occasionally.
  • No cause has been looked for, such as chromosome, fragile X or autoimmune conditions.
  • Donor egg treatment is recommended without checking the womb lining and hormone preparation first.
  • You have not been offered hormone replacement to protect bones and heart.

Questions à poser

  1. What is the likely cause of my ovarian insufficiency, and do I need genetic or autoimmune testing?
  2. Is there any chance of a pregnancy with my own eggs?
  3. Which hormone replacement do you suggest, and for how long should I take it?
  4. What does donor egg treatment involve, and what are success rates for women with POI?
  5. How often should my bones, heart and thyroid be checked?
  6. Could my sisters or daughters be affected?

Perspectives

Pronostic réaliste

POI is a lasting condition, but hormone replacement controls symptoms and protects bones and heart until about the natural age of menopause. Around 5 to 10 percent of women conceive naturally. Donor egg IVF gives pregnancy rates that mostly depend on the donor's age and the health of the womb lining, which is usually receptive in women on hormone replacement.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur insuffisance ovarienne prématurée

Can I ever get pregnant with my own eggs?

A small number of women with POI do conceive naturally, but it is unpredictable. For most, donor eggs offer the most reliable chance of pregnancy.

Do I need hormone replacement?

It is usually advised until around age 50 unless there is a medical reason to avoid it, as it protects bones, heart and brain health, not just symptoms.

What causes POI?

Causes include genetic conditions, autoimmune disease and chemotherapy or radiotherapy, but in many women no cause is found even after testing.

Is donor egg IVF the only option?

It is the most successful option for most, but adoption and parenting without a pregnancy are also choices. Egg freezing is not possible once ovaries have failed.

Clinical context

Working up insuffisance ovarienne prématurée properly

A condition like insuffisance ovarienne prématurée is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilité et fiv specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilité et fiv specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
