À propos de pediatric cleft lip
Ce qu'il faut savoir
Babies can have trouble with feeding and, if the gum is involved, with later tooth development. A cleft lip is seen at birth, and the team checks for other conditions. Ear and speech problems are more likely when the palate is also involved. The cleft does not close by itself.
Surgical repair is usually done at about three to six months, restoring the muscle and shape of the lip, and sometimes a nasal correction at the same time. Special bottles and sometimes a nasal moulding device help before surgery. A cleft team supports feeding, hearing, dental and speech care until adulthood. Families sometimes look abroad for experienced cleft teams.
Bilan avant le départ
Examens à déjà réalisés
- Photographs and the prenatal scan or birth examination notes describing whether the cleft is one-sided or two-sided, and whether the gum or palate is involved.
- Feeding and growth records, including the bottle or technique that has worked, and weight gains.
- Hearing assessment and ear examination, since ear fluid is more common in children with clefts.
- Any previous lip repair operation notes and the age at surgery.
- Genetic or syndrome assessment and a general health check, including heart screening where advised.
Deuxième avis
Quand obtenir un second regard
- Surgery is being delayed or suggested at an unusual age without a clear reason.
- The lip repair result is unsatisfactory and revision is being considered.
- The cleft is part of a syndrome or other anomalies and a full multidisciplinary review has not taken place.
- Different surgeons recommend different repair techniques and you want the reasoning compared.

Questions à poser
- Is the cleft complete or incomplete, and does it involve the gum or palate?
- When is the best time for lip repair, and what technique do you use?
- What feeding methods will help until the surgery?
- What further operations might be needed, such as nose, gum or jaw procedures?
- Which professionals will be part of my child's care team?
- How should the scar be looked after?
Perspectives
Pronostic réaliste
Children with cleft lip generally do very well. Repair gives a good appearance and normal lip function in most, with a fine scar that fades. Some children later need minor revision, nose refinement or dental and orthodontic care, and a few need gum bone grafting or jaw surgery in the teenage years if the jaw is affected. Outcomes depend on the cleft extent, surgical technique, and coordinated long-term team care.

Bilan diagnostique

When should the lip be repaired?
Usually at around three to six months of age, depending on the baby's health and the surgeon's protocol.
Will my child need more operations?
Possibly. Nose correction, gum grafting and, in some cases, jaw surgery may follow as the child grows.
Can my baby feed normally?
Most babies with cleft lip alone can breastfeed or bottle feed with some support; palate involvement makes it harder.
Will it happen again in another child?
The chance is slightly higher than in the general population, and genetic counselling can give a personal estimate.
Clinical context
Working up pediatric cleft lip properly
A condition like pediatric cleft lip is rarely well-served by jumping straight to treatment shopping. The sequence below is what a chirurgie esthétique specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or chirurgie esthétique specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
