À propos de ovarian endometrioma

Ce qu'il faut savoir

Symptoms include painful periods, pelvic pain, pain with sex and difficulty conceiving, although some women have none. Diagnosis is by pelvic ultrasound, with MRI sometimes used, and the typical appearance is distinctive. Endometriomas tend to recur and can reduce egg reserve, which blood tests such as AMH may reflect.

Hormonal treatment, such as the contraceptive pill or progestogens, controls pain and may stop growth, but it does not remove the cyst. Laparoscopic excision of the cyst wall is considered for pain, large cysts or when fertility treatment is planned, balancing the benefit against loss of ovarian tissue. IVF is an alternative for infertility. Patients sometimes travel abroad for specialist endometriosis surgery.

Bilan avant le départ

Examens à déjà réalisés

  • Pelvic ultrasound reports, including cyst size, number and whether it is on one or both ovaries, and an MRI if the picture is unclear.
  • AMH level and antral follicle count, which show ovarian reserve before any surgery.
  • Previous surgery notes, including any earlier endometriosis operations and findings.
  • CA-125 results, noting these can be raised in endometriosis, and tumour marker follow-up if indicated.
  • A pain and menstrual diary, and your fertility history including how long you have been trying.

Deuxième avis

Quand obtenir un second regard

  • Surgery is advised for a woman who wants pregnancy, and ovarian reserve may fall after cyst removal.
  • The cyst has grown, or has features on scan that look unusual.
  • You have had previous endometrioma surgery and it has come back.
  • IVF is being recommended and you want to know whether to operate first.

Questions à poser

  1. Is this definitely an endometrioma, and are there signs of deeper endometriosis?
  2. How might surgery affect my ovarian reserve?
  3. Is surgery better than hormone treatment or IVF for my goals?
  4. Which surgical technique best protects ovarian tissue?
  5. How likely is it to return, and how can recurrence be reduced?
  6. What happens if I leave it alone?

Perspectives

Pronostic réaliste

Endometriomas often recur unless suppressed with hormonal treatment after surgery. Excision relieves pain and may improve natural fertility in some women, but can reduce ovarian reserve, particularly with larger or bilateral cysts. Outcome depends on cyst size, age, ovarian reserve and associated endometriosis. Malignant change is rare. Care should be tailored to whether pain relief or pregnancy is the main aim.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur ovarian endometrioma

Is an endometrioma cancer?

No. It is benign, though the risk of ovarian cancer is slightly increased in women with endometriosis. Cysts with unusual features are checked more closely.

Will surgery affect my fertility?

It can lower egg reserve, because some healthy ovarian tissue may be removed with the cyst. This is why surgery is balanced carefully against alternatives in women who want children.

Can it come back?

Yes, recurrence is common without ongoing hormonal suppression. Contraceptive pills or progestogens after surgery reduce this risk in women not trying to conceive.

Do I need treatment if I have no symptoms?

Not always. Small, painless cysts can be monitored with ultrasound. Treatment is considered for growth, pain, fertility plans or unclear scan features.

Clinical context

Working up ovarian endometrioma properly

A condition like ovarian endometrioma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilité et fiv specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilité et fiv specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
