Normal Pressure Hydrocephalus

Normal pressure hydrocephalus is a build-up of cerebrospinal fluid in the brain's ventricles in older adults, without a marked rise in pressure. It is one of the few treatable causes of dementia-like symptoms.

À propos de normal pressure hydrocephalus

Ce qu'il faut savoir

The classic triad is a slow, shuffling, magnetic walk, memory and thinking problems, and urinary urgency or incontinence. Diagnosis is by MRI or CT showing enlarged ventricles, together with a trial of removing spinal fluid by lumbar puncture to see whether walking improves. Without treatment, symptoms usually worsen gradually.

The main treatment is a shunt, a thin tube that drains excess fluid from the brain to the abdomen. Patients who improve after the fluid-removal test are the most likely to benefit, and walking often responds best. Shunts can block or fail and need follow-up. Treatment abroad is not usually necessary, except for access to specialist neurosurgical assessment.

Bilan avant le départ

Examens à déjà réalisés

  • MRI or CT of the brain showing enlarged ventricles, with reports and earlier scans for comparison.
  • A description of your walking problem, ideally with a timed walking test or video before and after any tap test.
  • Results of a lumbar puncture tap test or extended lumbar drainage, and the change in walking afterwards.
  • Cognitive test scores and a record of bladder symptoms.
  • A list of other conditions and medicines, including blood thinners and any history of stroke or Parkinson's disease.

Deuxième avis

Quand obtenir un second regard

  • You are told the diagnosis is dementia or Parkinson's disease and NPH has not been considered.
  • A tap test was negative but your symptoms strongly suggest NPH.
  • Shunt surgery is advised and you want to understand the risks in older age.
  • A shunt was placed and symptoms have not improved or have returned.

Questions à poser

  1. How do my scans and symptoms support NPH rather than another cause?
  2. What did the tap test show, and does it predict benefit from a shunt?
  3. Which symptom is most likely to improve, and which least?
  4. What are the risks of a shunt, including infection, over-drainage and bleeding?
  5. What valve type is recommended, and how is it adjusted?
  6. What follow-up is needed over the long term?

Perspectives

Pronostic réaliste

Normal pressure hydrocephalus is one of the few treatable causes of walking difficulty and memory problems. Many patients, particularly those who respond to a tap test, improve after shunting, and walking improves most often. Benefit is greater when treated early and when the symptoms are mainly walking-related. Complications include over-drainage and shunt problems needing revision. Memory benefit is less predictable.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur normal pressure hydrocephalus

Is NPH the same as dementia?

No, though they can look similar and coexist. NPH is potentially reversible because the cause is fluid build-up, which a shunt can drain.

How is a shunt put in?

A thin tube is placed in the brain's fluid space and routed under the skin to the abdomen, where fluid is absorbed. The operation takes about one to two hours under general anaesthesia.

Will I recover fully after a shunt?

Many patients improve, particularly in walking, but full recovery is not guaranteed, especially if symptoms have been present for a long time.

What problems can a shunt cause?

Problems include infection, blockage, over-drainage with headache or bleeding around the brain, and valve problems. Valves are often adjustable, and regular reviews catch issues early.

Clinical context

Working up normal pressure hydrocephalus properly

A condition like normal pressure hydrocephalus is rarely well-served by jumping straight to treatment shopping. The sequence below is what a neurologie et neurochirurgie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or neurologie et neurochirurgie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
