Melanoma

Melanoma is a cancer of the pigment-producing cells of the skin. It can affect adults of any age and is the most serious common skin cancer because it can spread to lymph nodes and other organs if it is not removed early.

À propos de melanoma

Ce qu'il faut savoir

A mole that changes in size, shape or colour, has an irregular border, or becomes itchy, crusted or bleeding is the usual warning sign. Diagnosis is by removing the lesion for biopsy, and the thickness of the tumour under the microscope largely determines the outlook. Staging may include lymph node assessment and scans. Melanomas caught while thin are often cured by surgery alone.

Treatment starts with wide surgical excision of the lesion and a margin of normal skin, sometimes with sentinel lymph node biopsy. Mohs surgery is occasionally used for certain sites or subtypes. For higher-risk or advanced disease, immunotherapy and targeted drugs are added, and they have changed outcomes considerably. People travel abroad mainly for access to specific immunotherapy regimens or specialist oncology teams.

Bilan avant le départ

Examens à déjà réalisés

  • The original pathology report of the biopsy, including Breslow thickness, ulceration, mitotic rate and margin status.
  • Clear clinical photographs and a written description of the lesion's site and size before and after biopsy.
  • Imaging such as ultrasound of the lymph nodes, or CT or PET-CT for thicker or higher-stage disease, is usually requested.
  • Any sentinel lymph node biopsy report and the operation note from earlier surgery.
  • Mutation test results (such as BRAF) if the disease has spread or is at high risk of spreading.

Deuxième avis

Quand obtenir un second regard

  • The biopsy shows a thick, ulcerated or high-mitotic-rate melanoma and you are unsure whether a sentinel node biopsy is advised.
  • The pathology result is borderline or reported as an atypical melanocytic lesion, where pathologists often disagree.
  • Melanoma has spread to lymph nodes or other organs and you are weighing immunotherapy or targeted drugs.
  • Margins were reported as involved after the first excision and further surgery is recommended.

Questions à poser

  1. What is the exact stage of my melanoma, and what did the thickness and ulceration show?
  2. How wide should the surgical margin be at this site, and will I need a graft or flap?
  3. Do I need a sentinel lymph node biopsy, and what would the result change?
  4. Am I a candidate for adjuvant immunotherapy or targeted therapy, and what are the side effects?
  5. What happens if we remove only the lesion and do nothing further?
  6. How often will my skin and lymph nodes be checked afterwards, and for how many years?

Perspectives

Pronostic réaliste

Outlook depends mainly on thickness, ulceration and whether lymph nodes or other organs are involved. Thin melanomas found early are very often cured by surgery alone, while stage III and IV disease carries more risk of return. Modern immunotherapy and targeted drugs have improved results for advanced disease. Lifelong skin checks are needed, as a second melanoma can occur.

Bilan diagnostique

Is surgery always needed for melanoma?

Surgical removal is the main treatment for nearly all melanomas, with the margin depending on thickness. Very early melanoma in situ is treated by excision alone. Drug treatment is added mainly when lymph nodes or other organs are involved or the risk of return is high.

Can melanoma come back after treatment?

Yes. Risk depends on thickness, ulceration and node involvement. It can return at the scar, in nearby skin, lymph nodes or distant organs, and you can also develop a new melanoma elsewhere. This is why regular skin and node checks continue for years.

Is a sentinel node biopsy necessary?

It is usually discussed for melanomas above a certain thickness or with high-risk features. It stages the disease and guides further treatment. Thin melanomas often do not need it. Your surgeon should explain what the result would change for you.

Can I fly after melanoma surgery?

After a small excision, travel is usually possible once the wound is stable and your surgeon agrees. After lymph node surgery or a flap or graft, you may be advised to wait. Ask about wound care and swelling before you fly.

Clinical context

Working up melanoma properly

A condition like melanoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a oncologie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or oncologie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
