Macular Hole

A macular hole is a small break in the macula, the central part of the retina responsible for sharp detail vision. It usually affects people over 60, more often women.

À propos de macular hole

Ce qu'il faut savoir

Symptoms are blurred or distorted central vision, with straight lines looking bent and a dark or missing spot in the middle of sight. Side vision is unaffected. Diagnosis is by eye examination and optical coherence tomography (OCT), which shows the hole and its size. Without treatment, most full-thickness holes do not close and vision stays poor.

Surgery is the standard treatment: a vitrectomy removes the gel pulling on the retina, the thin inner membrane is peeled, and a gas bubble is placed to help the hole close. Patients may need to keep their head positioned for a few days. Most holes close, and vision improves, though cataract often develops afterwards. Small early holes are sometimes treated with an injection. Treatment abroad is sometimes considered for vitrectomy.

Bilan avant le départ

Examens à déjà réalisés

  • OCT scan report and images showing the hole's size and stage, which predicts the chance of closure.
  • Your recent visual acuity measurements and a description of distortion on an Amsler grid.
  • Duration of symptoms in weeks or months, as earlier surgery tends to give better results.
  • Records of previous eye surgery, especially cataract surgery, and any history of retinal detachment, high myopia or injury.
  • A list of medicines including blood thinners, and any condition affecting your ability to lie face-down if required.

Deuxième avis

Quand obtenir un second regard

  • You have been told to observe a hole that is getting larger, or has been present for many months.
  • Surgery is recommended and you want to compare the gas used and positioning requirements.
  • A previous operation failed to close the hole and you want to know whether a second procedure is worthwhile.
  • You have a very large or myopic macular hole, which may need special techniques.

Questions à poser

  1. What stage and size is my hole, and what does that mean for the chance of closing it?
  2. Is the hole likely to close on its own, or should I have surgery now?
  3. What type of gas will be used, and how long must I position my head?
  4. By what degree will my vision improve, and how long does recovery take?
  5. Are there limits on flying or altitude changes after gas is placed?
  6. What is the risk of cataract, or of the hole reopening?

Perspectives

Pronostic réaliste

Surgery closes most macular holes, often above ninety per cent for small to medium holes, and vision usually improves, though rarely to normal. Results are better when the hole is small and recent. Gas in the eye prevents flying and altitude change for weeks until it clears. Cataract often develops afterwards in eyes that still have their natural lens.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur macular hole

Can a macular hole close without surgery?

Some small holes, especially those that are recent, may close on their own, or with an injection in selected cases. Most larger holes need vitrectomy.

Do I need to lie face-down after surgery?

Often for a period, commonly several days, so the gas bubble presses on the hole. Some surgeons now use shorter positioning, so ask yours.

Can I fly after macular hole surgery?

Not while gas remains in the eye, which may take two to eight weeks, because altitude can raise eye pressure dangerously. Carry a wristband or note stating that gas is present.

Clinical context

Working up macular hole properly

A condition like macular hole is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ophtalmologie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ophtalmologie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
