Undescended Testis (Cryptorchidism)

Undescended testis, or cryptorchidism, is when one or both testes have not moved down into the scrotum. It is one of the most common birth differences in boys.

À propos de undescended testis (cryptorchidism)

Ce qu'il faut savoir

A doctor finds an empty scrotum on examination, and the testis may sit in the groin or abdomen. Many testes descend by themselves in the first three to six months, but if this has not happened by about six months, it is unlikely to. Ultrasound is not usually needed. Untreated, it increases the risk of reduced fertility and testicular cancer.

Hormone treatment is not generally recommended. The treatment is a small operation called orchidopexy to bring the testis down and fix it in the scrotum, ideally between about six and eighteen months of age. Testes in the abdomen may need keyhole surgery in stages. After surgery the boy needs ongoing examination, and teenagers should learn self-checks. Treatment abroad is seldom needed.

Bilan avant le départ

Examens à déjà réalisés

  • A paediatric surgeon's or urologist's examination note stating whether the testis is felt in the groin or not felt at all, and which side.
  • Scrotal and groin ultrasound results, if done, though they cannot always find a testis.
  • The child's age, birth history (including prematurity) and any other birth differences, such as hypospadias.
  • Hormone tests if both testes are undescended, or if the genitalia appear atypical.
  • Anaesthetic fitness: weight, vaccination status, and any previous operations.

Deuxième avis

Quand obtenir un second regard

  • Your child is older than about 6 to 12 months and has not yet been referred for surgery.
  • A testis cannot be felt on either side, and no specialist has examined your child.
  • A previous orchidopexy has failed, with the testis pulling back up.

Questions à poser

  1. Is the testis in the groin, the abdomen or not found, and does it need a laparoscopy?
  2. At what age should the operation be done?
  3. What is the effect on fertility and cancer risk, and how will we monitor later?
  4. What are the risks of surgery, such as testicular shrinkage?
  5. When should my child return to normal activity?

Perspectives

Pronostic réaliste

Orchidopexy in infancy brings the testis into the scrotum in most children with a very low complication rate. Earlier surgery gives the best chance of preserving fertility, which is reduced more when both testes are affected. Risk of testicular cancer is higher than average even after surgery, so self-examination is taught from adolescence. Testes lying in the abdomen are harder to treat and may need staged surgery.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur undescended testis (cryptorchidism)

Will an undescended testis come down by itself?

Some descend in the first three to six months of life. If it has not descended by about six months, it rarely will and surgery is advised.

Does an undescended testis affect fertility?

It can reduce fertility, especially if both sides are affected or surgery is late. Early orchidopexy gives the best chance of preserving it.

Is the operation a day case?

Often yes. Most children go home the same day and return to normal play within a week or two, with advice to avoid straddle activities initially.

Clinical context

Working up undescended testis (cryptorchidism) properly

A condition like undescended testis (cryptorchidism) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a chirurgie pédiatrique specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or chirurgie pédiatrique specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
