Chronic Pancreatitis

Chronic pancreatitis is long-standing inflammation that scars the pancreas and gradually reduces its function. Alcohol is the most common cause, though genetic, autoimmune and blockage-related causes also occur.

À propos de chronic pancreatitis

Ce qu'il faut savoir

The main symptoms are upper abdominal pain that may spread to the back, weight loss, oily pale stools from poor digestion and, in time, diabetes. CT, MRI with MRCP or endoscopic ultrasound shows scarring, calcification and duct changes. The condition tends to progress, so stopping alcohol and smoking are central to slowing it.

Treatment includes stopping alcohol and smoking, pain relief, pancreatic enzyme capsules with meals and diabetes management. When a blocked or narrowed pancreatic duct causes pain, ERCP with stenting or stone removal can help. Surgery to drain the duct or remove part of the pancreas is considered when endoscopic treatment fails. Treatment abroad may be considered for advanced endoscopic or pancreatic surgery.

Bilan avant le départ

Examens à déjà réalisés

  • CT or MRI with MRCP showing duct size, stones, calcification, cysts or any mass in the pancreas.
  • Endoscopic ultrasound report if done, particularly if a mass is suspected.
  • Pancreatic function tests such as faecal elastase, and blood sugar or HbA1c results.
  • A list of pain medicines and enzyme supplements, including doses, and a record of alcohol and smoking history.
  • Reports of previous ERCP, stenting, drainage or surgery, and any hospital admissions for flare-ups.

Deuxième avis

Quand obtenir un second regard

  • You have constant pain on strong medicines and have not been assessed for endoscopic or surgical options.
  • Imaging shows a new mass or rapidly worsening duct changes, and cancer has not been excluded.
  • You have been advised to have surgery, and the type of operation does not match your duct anatomy.

Questions à poser

  1. Where is the blockage or disease in my pancreas, and what is causing the pain?
  2. Is an endoscopic procedure enough, or is an operation likely to give longer relief?
  3. How can cancer be excluded in my case?
  4. Which operation, such as drainage, removal of part of the pancreas, is suited to my scans?
  5. How will digestion and blood sugar be managed afterwards, and what lifestyle changes do I need?

Perspectives

Pronostic réaliste

Chronic pancreatitis cannot be reversed, but pain, complications and nutrition can often be controlled. Stopping alcohol and tobacco slows progression. Endoscopic or surgical treatment relieves pain in many patients with duct obstruction, though results vary and some pain may persist. Enzyme replacement and diabetes care are usually needed over the long term, and the risk of pancreatic cancer is somewhat raised.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur chronic pancreatitis

Will chronic pancreatitis get better if I stop drinking?

Stopping alcohol and smoking slows the damage and can reduce pain and flare-ups, though existing scarring does not reverse. It is one of the most useful steps.

Do I need surgery?

Not for everyone. Many patients are managed with pain medicines, enzymes and diet. Endoscopic or surgical treatment is considered for persistent pain with duct blockage, cysts or suspected tumours.

Will I develop diabetes?

Many people with long-standing chronic pancreatitis eventually develop diabetes as insulin-making cells are lost, so blood sugar is checked regularly.

Does chronic pancreatitis cause cancer?

It raises the risk modestly over many years, particularly with smoking. A new mass, weight loss or rapid change in symptoms should be investigated.

Clinical context

Working up chronic pancreatitis properly

A condition like chronic pancreatitis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a chirurgie digestive specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or chirurgie digestive specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
