À propos de anorectal malformation
Ce qu'il faut savoir
It is usually found at birth when no normal anal opening is seen, and other differences of the spine, kidneys, heart or gut are looked for because they often occur together. Diagnosis is by examination, ultrasound and X-ray or MRI studies. Severity ranges from low defects with good bowel control to complex ones with lasting continence difficulties.
Treatment is surgical reconstruction. Some babies need a temporary colostomy first, then definitive repair, most often posterior sagittal anorectoplasty (PSARP), followed by dilatation of the new opening and later closure of the colostomy. Long-term care covers bowel management, toilet training and urinary follow-up. Families sometimes seek surgeons with large experience of these rare conditions, including abroad.
Bilan avant le départ
Examens à déjà réalisés
- Operative notes of any colostomy and previous repair, including the type of malformation (low, intermediate or high).
- Distal colostogram or contrast study images showing the level of the rectum and any fistula.
- Imaging of the spine, sacrum, kidneys and heart, since associated anomalies are common (VACTERL).
- Bowel function diary including soiling, constipation and continence aids used.
- Urological assessment, including renal ultrasound and any urinary tract infections.
Deuxième avis
Quand obtenir un second regard
- Repair is planned at a centre that rarely manages anorectal malformations.
- Your child has persistent soiling or constipation after repair and the cause has not been identified.
- Associated anomalies of the kidney, spine or heart have not been fully assessed.
- A redo operation is proposed and you want to know why the first repair has not worked.

Questions à poser
- What type of malformation is this, and which repair does it need?
- Are other organs involved that need checking?
- What bowel function can we realistically expect as a child grows?
- Is a bowel management programme available, and who will supervise it?
- When will the colostomy be closed?
- What follow-up is needed through childhood and into adult life?
Perspectives
Pronostic réaliste
Most children with anorectal malformation are able to survive and lead full lives after repair. Bowel control depends on the type of defect, the spine and sacrum, and associated anomalies. Those with low defects usually have good control, whereas those with high defects often need long-term bowel management. Constipation is the most common problem, and ongoing specialist follow-up is important.

Bilan diagnostique

04 · FAQ
Questions fréquentes sur anorectal malformation
Will my child be able to control their bowels?
It varies. Children with low defects usually achieve control, while those with higher defects may need a bowel management programme throughout childhood.
Does a colostomy stay forever?
Usually not. A temporary colostomy is often closed after the main repair has healed, typically after a few months.
Are other problems linked to this condition?
Often. Spine, kidney, heart and limb anomalies can accompany it, so screening tests are routinely requested.
Clinical context
Working up anorectal malformation properly
A condition like anorectal malformation is rarely well-served by jumping straight to treatment shopping. The sequence below is what a chirurgie pédiatrique specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or chirurgie pédiatrique specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
