Ankle Arthritis
Ankle arthritis is wear of the cartilage in the ankle joint, causing pain and stiffness. It most often develops years after an old fracture or repeated ligament sprains, rather than from ageing alone.

À propos de ankle arthritis
Ce qu'il faut savoir
Pain on walking, swelling, stiffness and a feeling of grinding are typical, and walking on uneven ground is hardest. Diagnosis is by examination and weight-bearing X-rays, with CT or MRI to plan treatment. The condition progresses at different rates, and symptoms can fluctuate.
Initial treatment includes pain relief, supportive footwear, bracing, activity changes, weight control and sometimes steroid injections. When these no longer help, surgery depends on age, activity and alignment: arthroscopic cleaning, bone realignment, ankle fusion (arthrodesis) which gives reliable pain relief but a stiff joint, or ankle replacement which preserves movement. Treatment abroad is mainly considered for replacement.
Bilan avant le départ
Examens à déjà réalisés
- Weight-bearing X-rays of the ankle and foot, plus the hindfoot alignment view.
- CT or MRI if cartilage damage, bone cysts or neighbouring joint arthritis need to be mapped.
- Records of previous fractures, ligament injuries or infection around the ankle.
- Details of treatments tried, such as insoles, braces, injections and physiotherapy.
- Information on bone quality, diabetes, smoking and circulation, which affect surgical outcome.
Deuxième avis
Quand obtenir un second regard
- Ankle fusion or replacement is advised without trying bracing, footwear changes or injections.
- You are young and active and want to compare replacement with fusion.
- Alignment of the foot is abnormal, which may need correcting at the same time.
- Neighbouring joints such as the subtalar joint are also arthritic, which affects the choice of operation.

Questions à poser
- Is my arthritis limited to the ankle, or are the nearby joints involved?
- Would I be better with replacement or fusion, given my age and activity?
- How long does an ankle replacement last, and what happens if it wears out?
- Do I need any alignment correction at the same time?
- How long will I be off my foot, and when can I fly?
- What activities will I be able to return to?
Perspectives
Pronostic réaliste
Ankle arthritis is usually managed for years with simple measures. When surgery is needed, both fusion and replacement relieve pain in most people, though fusion limits movement and may strain neighbouring joints over time. Replacement keeps movement but has a limited lifespan and may need revision. Results depend on age, alignment, bone quality and activity demands.

Bilan diagnostique

Is ankle replacement as good as hip or knee replacement?
Ankle replacements relieve pain well and preserve movement, but they are less long-lasting and revision is more common. Fusion is more durable but stiff.
Do I need surgery?
Not necessarily. Bracing, footwear, activity changes and injections help many people for years. Surgery is considered when pain limits daily life despite these.
How long is recovery?
Expect about six weeks without weight on the foot after fusion or replacement, followed by rehabilitation. Return to full activity takes several months.
Clinical context
Working up ankle arthritis properly
A condition like ankle arthritis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a orthopédie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or orthopédie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
