Adnexal (Ovarian) Mass
An adnexal mass is a lump found on or near the ovary or fallopian tube. Most are harmless fluid-filled cysts, but a small number are tumours, so the key question is whether the mass is benign or suspicious.

À propos de adnexal (ovarian) mass
Ce qu'il faut savoir
Many are found by chance on a scan; others cause pelvic pain, bloating or pressure, and a twisting ovary can cause sudden severe pain. Pelvic ultrasound describes the appearance, MRI and the CA-125 blood test help in older women, and risk scoring systems decide how worrying it is. Simple cysts in younger women often disappear on their own.
Simple or small cysts are usually watched with repeat scans. Cysts that persist, grow, cause pain or look suspicious are removed, ideally by keyhole surgery that conserves the ovary when possible. Suspicious masses are managed by a gynaecological oncology team, sometimes with more extensive surgery. Treatment abroad is generally considered only for planned, non-urgent surgery.
Bilan avant le départ
Examens à déjà réalisés
- Pelvic ultrasound report using standard descriptors (size, solid parts, septations, blood flow), ideally with an IOTA or O-RADS risk score.
- MRI or CT report if the ultrasound was unclear or if spread is suspected.
- Tumour markers such as CA-125, and in younger women AFP, hCG and LDH, with dates.
- Your age, menopausal status, family history of ovarian or breast cancer and any BRCA result.
- Previous scans showing whether the mass is new, stable or growing.
Deuxième avis
Quand obtenir un second regard
- Surgery including removal of both ovaries is advised for a mass that looks benign, especially if you wish to preserve fertility.
- Scans suggest suspicious features and you want an assessment by a gynaecological oncology team before any operation.
- The mass has grown or changed on repeat imaging.
- You are a teenager or young woman and an ovary-sparing approach has not been discussed.

Questions à poser
- What do the scans suggest about the chance that this mass is cancer?
- Can the cyst be removed while keeping the ovary?
- Is a gynaecological oncologist needed for this operation?
- Could a cyst be left and watched, and how often would I be rescanned?
- How would removal of an ovary affect my hormones and fertility?
- Will the removed tissue be sent for analysis, and when will I get results?
Perspectives
Pronostic réaliste
Most adnexal masses are benign, especially in younger women, and many simple cysts resolve or stay stable. Cyst removal usually restores normal ovarian function when the ovary is preserved. If a tumour is malignant, the outlook depends on type and stage at diagnosis, and early-stage disease has a good prognosis. Risk assessment before surgery guides the extent of the operation.

Bilan diagnostique

Does an ovarian mass mean cancer?
Usually not. Most are benign cysts or growths, particularly in younger women. Ultrasound features, tumour markers and age help estimate the risk.
Can I keep my ovary?
Often yes, if the mass appears benign. Cystectomy removes the cyst and leaves normal ovarian tissue, which is especially important if you want to have children.
Do all masses need surgery?
No. Simple cysts often resolve or are monitored with repeat ultrasound. Surgery is considered for larger, persistent, painful or suspicious masses.
Clinical context
Working up adnexal (ovarian) mass properly
A condition like adnexal (ovarian) mass is rarely well-served by jumping straight to treatment shopping. The sequence below is what a gynécologie specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or gynécologie specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
