About TURP (Transurethral Resection of Prostate)
Under spinal or general anaesthesia, the surgeon passes a resectoscope through the urethra and shaves the inner prostate away in small chips, sealing blood vessels with current. Bipolar versions use saline and lower the risk of fluid absorption. The chips are washed out and sent for pathology, and a catheter with irrigation is left in for a day or two.
Most men get a much stronger stream and a lasting improvement. Outcomes depend on the gland size, how obstructed the bladder is, bladder muscle function and the surgeon's experience. Dry ejaculation is very common. Bleeding, urethral narrowing and, uncommonly, temporary incontinence can occur, and very large glands are usually better treated with enucleation.
Expect to stay two to three nights until the urine is clear and the catheter is out. Remain near the hospital for a few more days and avoid heavy lifting for several weeks, as late bleeding can occur. Bring your PSA, flow tests and medicine list, particularly blood thinners, and arrange a follow-up with a urologist at home.
Usually a good fit if
- Moderate to severe urinary symptoms from BPH not controlled by tablets.
- Urinary retention, bladder stones, repeated infections or kidney strain due to the prostate.
- A prostate of small to moderate size (often under about 80 mL).
- A man fit for spinal or general anaesthesia.
May need to wait if
- Suspected prostate cancer needs investigation before treatment.
- Active urinary infection must be treated first.
- Very large prostates may be safer with HoLEP or open surgery.
- Uncorrected bleeding disorders or blood thinners need a plan from your doctor.
How to prepare
- Bring your PSA, prostate ultrasound, flow test and urine culture; any infection should be cleared first.
- Tell your doctor about blood thinners, aspirin and other medicines; changes only on the doctor's instruction.
- Expect blood tests and an anaesthetic review, especially with heart or lung disease.
- Discuss retrograde ejaculation, leakage and the chance of repeat surgery before the operation.
- Follow fasting instructions and plan a hospital stay of a few days.
Questions to ask your surgeon
- Is TURP right for my prostate size, or would HoLEP, GreenLight or Rezum be better?
- Will you use monopolar or bipolar TURP, and why?
- How many TURPs do you perform each year, and what are your bleeding and re-operation rates?
- How long will I have a catheter and stay in hospital?
- What are the risks to continence, ejaculation and erections?
- Who reviews my pathology and my stream when I am home?
Red flags when comparing hospitals
- Offered without a PSA test or consideration of cancer.
- No discussion of alternatives or of dry ejaculation risk.
- Surgeon does few prostate resections and has no blood transfusion backup.
- No plan for catheter removal and follow-up.
