About Toric IOL Implantation
The toric lens is implanted during standard cataract surgery, a day case under local or topical anaesthesia with light sedation. The surgeon marks the axis of astigmatism, makes a 2 to 3 mm corneal incision, removes the cloudy lens and rotates the toric lens to the exact angle in the capsule. Precise alignment is what makes it work.
A toric lens can greatly reduce astigmatism, which a standard lens leaves uncorrected, and many people see clearly at distance without glasses. Results depend on accurate measurements, the lens staying aligned and a regular corneal shape. If the lens rotates, a small adjustment may be needed, and reading glasses are usually still required.
Plan about a week for the first-day and one-week checks, which matter because early lens rotation can be corrected. Bring your glasses prescription and any corneal scans. Avoid rubbing the eye. Arrange a review at home after about a month to confirm the final prescription.
Usually a good fit if
- Cataract with at least moderate regular corneal astigmatism.
- Wanting good distance vision with less dependence on glasses.
- Healthy macula and stable, regular cornea.
- Able to attend early reviews to check lens position.
May need to wait if
- Irregular astigmatism such as keratoconus.
- Zonular weakness or a capsule that cannot hold the lens steady.
- Severe dry eye or macular disease.
- Unrealistic expectations of glasses-free vision.
How to prepare
- Complete biometry, corneal topography and tomography to confirm regular astigmatism; stop contact lens wear beforehand.
- Bring prior eye history, including laser surgery or corneal disease.
- Treat dry eye before measurements.
- Review medicines with your doctor; do not stop any without instruction.
- Arrange transport and early reviews.
Questions to ask your surgeon
- What amount of astigmatism will the lens correct and how is the power calculated?
- How will the lens axis be aligned during surgery?
- What is the chance of the lens rotating and needing repositioning?
- Will I need glasses for reading?
- Could a laser touch-up be needed?
- Who will do follow-up at home?
Red flags when comparing hospitals
- No corneal tomography before choosing a toric lens.
- Not mentioning the possibility of rotation and repositioning.
- Promising total glasses independence.
- No record of lens power and axis.
