About RNS (Responsive Neurostimulation)
Responsive neurostimulation implants a device in the skull that detects abnormal brain activity and delivers brief electrical pulses to prevent seizures. It is for drug-resistant focal epilepsy.
Electrodes are placed on or in the brain where seizures begin, and a small neurostimulator sits in a recess in the skull. The device records brain activity continuously and responds when it recognises a seizure pattern. Doctors review the recordings and adjust detection and stimulation settings during clinic visits.
Usually a good fit if
- Drug-resistant focal epilepsy with one or two identified seizure foci.
- Seizures arising from areas where resective surgery would risk speech, vision or memory.
- Patients not suitable for, or not wanting, removal surgery.
- Those able to attend follow-up for programming.
May need to wait if
- Generalised epilepsy without a focal source.
- Seizures from more than two foci.
- Seizures that are not epileptic.
- Active infection or uncorrected bleeding disorder.
How to prepare
- Bring EEG reports, MRI and a list of anti-epileptic medicines tried with doses.
- Do not change anti-epileptic doses unless your neurologist instructs you.
- Expect MRI, video-EEG review and an assessment of how the seizure focus is identified.
- Tell the team about other implants, as MRI conditions apply to the device.
- Plan for several programming visits and for regular uploads of data after surgery.
Questions to ask your surgeon
- Why is RNS better for me than resection or VNS?
- Where will the leads be placed, and what is the target?
- How many RNS implants does your team do each year?
- What results do patients like me typically experience over several years?
- How is data downloaded and who adjusts the settings at home?
- What is the battery life, and how is it replaced?
Red flags when comparing hospitals
- No full epilepsy evaluation to locate seizure foci.
- No plan for long-term programming and data review.
- Promises of a seizure-free outcome.
- No explanation of MRI limits.
