About Brain Aneurysm Coiling (Endovascular)
Brain aneurysm coiling treats an aneurysm from inside the artery, filling it with soft platinum coils so blood can no longer enter. It avoids opening the skull and may be used before or after a rupture.
A catheter is passed from an artery at the groin or wrist up to the brain, and a tiny tube is guided into the aneurysm under X-ray. Coils are packed in until the sac is closed off. Wide-necked aneurysms may need a balloon or a stent to hold the coils in place. Clipping is the alternative, and the aneurysm's shape often decides which is better.
Usually a good fit if
- A ruptured aneurysm suitable for coiling.
- An unruptured aneurysm with a narrow neck that coils can retain.
- Older patients or those with other illnesses who may tolerate surgery poorly.
- Aneurysms at the back of the brain that are hard to reach surgically.
May need to wait if
- Very wide-necked aneurysms where coils might not stay in place.
- Allergy to contrast that has not been prepared for.
- Inability to take antiplatelet medicine if a stent is needed.
- Aneurysm better treated by clipping because of branch vessels.
How to prepare
- Bring CT angiogram or catheter angiogram images on disc and any report of earlier bleeding.
- Review blood thinners, aspirin and clopidogrel with your doctor, since stent use may require antiplatelet medicine.
- Tell the team about contrast dye allergy and kidney problems.
- Expect blood tests and a clotting check before the procedure.
- Ask if a stent might be needed, because that determines how long you must take antiplatelet drugs.
Questions to ask your surgeon
- Is coiling, clipping or a flow diverter best for my aneurysm?
- Will a balloon or stent be needed, and which blood thinner for how long?
- How often do aneurysms like mine reopen after coiling?
- What is the plan if the aneurysm is only partly closed?
- What follow-up scans are required, and when?
- Can my local hospital do the follow-up angiograms?
Red flags when comparing hospitals
- No neurosurgeon available if a complication needs surgery.
- No biplane angiography suite or experienced neuro-interventionalist.
- No plan for follow-up imaging.
- Stent advice without any explanation of blood thinner duration.

Can coiled aneurysms come back?
Sometimes the coil mass settles and the aneurysm partly reopens, so follow-up imaging is important. Further coiling or surgery may be advised.
Do I need blood thinners?
If a stent or balloon was used, usually yes for a defined period, only as your doctor instructs. Without a stent, requirements are different.
Is coiling less risky than surgery?
Recovery is faster, but both have risks. Choice depends on the aneurysm and your health. Your team can compare them for you.
When can I fly?
Often within one to two weeks if stable, but confirm with your team, particularly after a ruptured aneurysm.
