About Pterygium Excision + Conjunctival Autograft
The surgery is a day case under local anaesthesia. The surgeon carefully lifts and removes the pterygium from the cornea and conjunctiva, then takes a small graft of healthy conjunctiva from under the upper eyelid. The graft is fixed over the bare area with fibrin glue or fine stitches, and the eye is covered with a patch.
The main benefit is relief of irritation and redness, clearer vision where the growth was distorting the cornea, and a better appearance. The graft technique greatly reduces recurrence compared with simple removal. Regrowth is more likely in younger people and in those with strong sun exposure, so UV protection matters.
Expect some redness and discomfort for one to two weeks while the graft heals. Plan to stay about a week for the first checks, and use anti-inflammatory drops as prescribed. Bring your eye records, wear sunglasses and a hat outdoors, and arrange a review at home after a month to check for early recurrence.
Usually a good fit if
- Pterygium encroaching on the cornea and affecting vision.
- Persistent irritation, redness or recurrent inflammation.
- Astigmatism or visual distortion caused by the growth.
- Cosmetic concerns after discussion with your surgeon.
May need to wait if
- Small pterygia that are not growing, where observation may do.
- Active eye infection.
- Severe dry eye, which should be treated first.
- Bleeding tendency not yet managed by your doctor.
How to prepare
- Bring your eye history, including previous pterygium surgery or glaucoma.
- Review medicines, especially blood thinners, with your doctor; do not stop them yourself.
- Ask whether glue or stitches will be used, and about the recovery.
- Arrange transport; the eye will be patched.
- Plan to avoid dust, wind and sunlight afterwards.
Questions to ask your surgeon
- Will you use a graft, and with stitches or glue?
- How often does the pterygium return after your technique?
- Will mitomycin C be used, and what are its risks?
- How long will my eye be red and irritated?
- How will I protect the eye from sun and dust afterwards?
- When should I be checked for regrowth?
Red flags when comparing hospitals
- Excision with no graft, which has a high regrowth rate.
- No discussion of recurrence.
- Using chemicals such as mitomycin without explaining risks.
- No follow-up for regrowth.

Is the surgery painful?
Mild discomfort for a couple of days with irritation if sutures are used.
Does it improve vision?
It may reduce astigmatism and improve clarity if the growth was affecting the cornea.
