About Brachytherapy (Prostate Seed Implantation)
Under spinal or general anaesthesia, and guided by ultrasound, the specialist places seeds through needles passed between the legs, following a plan worked out from scans. Typically around fifty to a hundred small seeds are used, and they give off radiation over weeks to months. It is mainly used for low and intermediate risk cancer, sometimes combined with external beam radiotherapy.
Cancer control for suitable patients is comparable to surgery or external radiotherapy. Outcomes depend on the tumour risk group, prostate size, the quality of the seed placement and the team's experience. Urinary frequency and discomfort are common for some months, and some men have lasting bowel or erectile effects. Regular PSA tests follow for years.
Most men go home the same day, sometimes with a catheter. Radiation precautions apply for a few weeks or months, such as keeping a distance from pregnant women and young children, and the hospital will give a card to carry for travel and airport security. Bring your biopsy report, MRI and PSA history, and arrange PSA follow-up at home.
Usually a good fit if
- Low- to favourable intermediate-risk prostate cancer confined to the gland.
- A prostate of suitable size, and good urinary flow, to reduce urinary side effects.
- A man who prefers a single short procedure to weeks of external-beam radiotherapy.
- Fit for spinal or general anaesthesia.
May need to wait if
- Very large prostates, or severe urinary symptoms, increase the risk of urinary retention and may need other treatment.
- Previous prostate surgery (TURP) or pelvic radiation can make seed placement difficult.
- High-risk or spread cancer needs other treatment, as advised.
- Inflammatory bowel disease raises the risk of radiation bowel problems.
How to prepare
- Bring your biopsy report, PSA, prostate MRI and any bone scans so the team can confirm that the cancer is localised.
- Expect a planning ultrasound to measure the prostate and plan seed placement.
- Review blood thinners and urinary medicines with your doctor; changes only on the doctor's instruction.
- Discuss the radiation safety rules for close contact with children and pregnant women for the first weeks.
- Plan to remain nearby for early reviews and arrange follow-up PSA tests at home.
Questions to ask your surgeon
- Is brachytherapy suitable for my risk group, and should I have additional treatment?
- How many implants has your team done, and which isotope will be used?
- What are the risks to urinary flow, bowels and erections?
- What radiation precautions do I need at home, and for how long?
- How will you verify the seed positions and measure success?
- Who will follow my PSA regularly in my home country?
Red flags when comparing hospitals
- Promotion of brachytherapy for high-risk or spread disease without discussing other options.
- No MRI, biopsy or staging work-up beforehand.
- No radiation physicist or radiation oncologist involved in dose planning and checks.
- No plan for radiation safety or follow-up PSA.
