About Microvascular Decompression (MVD)
Microvascular decompression moves a blood vessel away from the trigeminal nerve, relieving severe facial pain from trigeminal neuralgia without damaging the nerve. It is for people whose pain no longer responds to medicine.
Under general anaesthesia, a small opening is made behind the ear. The surgeon uses a microscope to find the point where an artery or vein presses on the nerve at the brainstem and places a tiny Teflon pad between them. The nerve is left intact, which is why feeling in the face is usually preserved.
Usually a good fit if
- Classical trigeminal neuralgia with MRI showing a vessel touching the nerve.
- Pain that no longer responds adequately to carbamazepine or similar drugs, or causes side effects.
- Reasonable general fitness for general anaesthesia.
- Patients wanting a durable treatment that avoids numbness.
May need to wait if
- Pain that is constant rather than stabbing, which may not respond.
- Multiple sclerosis-related pain, where results are less certain.
- Frailty making anaesthesia unsafe.
- Active infection or bleeding disorder.
How to prepare
- Bring MRI scans on disc, ideally high-resolution sequences, and details of medicines tried, such as carbamazepine.
- Do not stop carbamazepine or other pain medicines unless your doctor instructs you, and ask how they will be reduced after surgery.
- Expect blood tests, including sodium and liver function if you take carbamazepine.
- Describe pain triggers clearly to the team.
- Ask for a cardiology or anaesthetic review if you have other illnesses.
Questions to ask your surgeon
- Does my MRI show a vessel in contact with the nerve?
- How many MVD operations do you perform each year?
- What is the chance of long-term pain relief, and of recurrence?
- What are the risks to hearing, facial feeling and spinal fluid leak?
- Are alternatives, such as radiosurgery, better for me?
- How will my pain medicines be reduced?
Red flags when comparing hospitals
- Surgery recommended without a proper trigeminal neuralgia diagnosis and MRI.
- No experience with microscope-based skull base surgery.
- Promise of 100 percent permanent relief.
- No explanation of alternatives to surgery.
