About Lingual Braces

Custom-made brackets are fixed to the tongue side of the teeth, and an archwire is adjusted at regular visits to move them. Because the brackets are individually fabricated, treatment takes longer to plan and needs a clinician trained in lingual work. Typical treatment is 18 to 30 months.

Lingual braces can treat many problems, including complex ones, but they can feel bulky at first and affect speech for a couple of weeks. Results depend on the case, careful hygiene and the orthodontist's skill. Tongue irritation usually eases within a few weeks, and retainers are needed afterwards.

Adjustments are needed every four to eight weeks, so planning a course abroad requires a local orthodontist who is willing to continue treatment. Agree this before travelling and bring full records. Expect to stay several days for fitting, and carry orthodontic wax and a spare toothbrush kit.

Usually a good fit if

  • You have crowding, spacing or bite problems and want hidden braces.
  • Your teeth and gums are healthy.
  • You can commit to regular adjustments and cleaning.
  • Your teeth are long enough for brackets on the inside.

May need to wait if

  • Very short teeth or deep bites that make lingual brackets difficult.
  • Poor oral hygiene or untreated gum disease.
  • Inability to attend regular appointments.
  • Severe jaw discrepancy needing surgery.
How to prepare
  1. Have a dental check-up and treat decay and gum disease first.
  2. Bring previous X-rays and expect scans, photographs and possible extractions.
  3. Tell your dentist about allergies, medical conditions and medicines.
  4. Buy wax, an interdental brush and soft food supplies for the first weeks.
  5. Ask how adjustment visits and emergencies will be handled if you live abroad.
Questions to ask your surgeon
  1. Are lingual braces suitable for my bite and tooth shape?
  2. Are brackets custom-made, and from which system?
  3. How long will my tongue irritation last?
  4. How will my speech be affected?
  5. What if a bracket breaks at home?
  6. Can a local orthodontist continue the treatment?
Red flags when comparing hospitals
  • Fitting by a dentist without lingual orthodontic training.
  • No custom brackets or planning.
  • No explanation of speech and tongue irritation.
  • No retainers in the plan.
