About GreenLight Laser Prostatectomy

The surgeon passes a scope through the urethra and sweeps laser light across the enlarged prostate, which absorbs the energy and vaporises. Spinal or general anaesthesia is used and the operation commonly takes less than an hour. Because the laser seals small vessels as it works, it is often chosen for men who take blood thinners. A catheter is left for a short time.

Most men notice a stronger urine stream within days to weeks and need fewer or no prostate tablets. Outcomes depend on the prostate size, the laser technique and the surgeon's experience. Vaporising leaves little tissue for pathology, and very large prostates may be better suited to enucleation. Dry ejaculation is common, and some men need retreatment years later.

Many patients go home the day after surgery, often still with a catheter for a day or two. Plan to stay near the hospital until it is out and you are passing urine well. Expect urinary urgency and some blood in the urine for a few weeks. Bring your medication list, especially blood thinners, and recent PSA and flow test results.

Usually a good fit if

  • Troublesome urinary symptoms from BPH, such as weak stream, hesitancy and getting up at night, despite medicines.
  • Urinary retention or bladder stones caused by an enlarged prostate.
  • Men on blood thinners who are at higher risk of bleeding with other prostate operations.
  • Prostate within the size range the surgeon can treat with this laser.

May need to wait if

  • Suspected prostate cancer or a high PSA needs to be investigated before treatment.
  • Active urinary infection needs clearing first.
  • Very large prostates may be better suited to enucleation techniques such as HoLEP.
  • Medical risks from the anaesthetic need to be optimised.
How to prepare
  1. Bring your prostate size (ultrasound), PSA, urine flow tests and recent urine culture; infection must be cleared first.
  2. Tell the doctor about blood thinners, aspirin and other medicines, including those for blood pressure and the prostate; changes only on the doctor's instruction.
  3. Expect blood tests, an ECG if needed, and anaesthetic review, especially if you have heart or lung disease.
  4. Follow fasting instructions and plan to rest and avoid heavy lifting afterward.
  5. Discuss sexual function and fertility, as retrograde ejaculation is common.
Questions to ask your surgeon
  1. Is my prostate size suitable for GreenLight, or would HoLEP or TURP suit better?
  2. How long will I need a catheter, and how long will I stay in hospital?
  3. Will tissue be sent for pathology, given vaporisation leaves little tissue?
  4. What are the risks to ejaculation, bleeding, urgency and stricture?
  5. How many GreenLight procedures do you perform and how many have needed repeat surgery?
  6. Who can remove the catheter or review me back home?
Red flags when comparing hospitals
  • Offered to men with suspected cancer without proper PSA, examination or biopsy work-up.
  • Claims that sexual side effects or retrograde ejaculation do not happen.
  • No discussion of other options for larger prostates.
  • No plan for pathology or follow-up of blood in the urine.
