About Endodontic Retreatment
The dentist removes the crown or filling as needed, takes out the old root filling, and re-cleans and disinfects the canals, often under a microscope. They look for missed canals, cracks or leaks. The canals are then refilled and sealed, and the tooth is restored with a filling or new crown.
Success depends on why the first treatment failed, the tooth's anatomy, the presence of a root fracture and the quality of the final seal. Many retreated teeth heal and last for years, but a cracked tooth may still need extraction or root-end surgery. Healing is checked on X-rays after several months.
Retreatment may be completed in one or two visits, so a short stay of a few days usually suffices, with extra time if a new crown is needed. Mild soreness is common. Bring previous X-rays and treatment notes, and arrange a dental review at home for follow-up imaging.
Usually a good fit if
- A root-treated tooth with persistent pain, swelling or an infection on X-ray.
- The tooth is restorable and the roots are not cracked.
- You want to keep your natural tooth.
- The tooth has enough bone support.
May need to wait if
- A vertical root fracture, which usually means extraction.
- The tooth cannot be restored or has severe gum disease.
- Uncontrolled diabetes or immune problems that slow healing.
- A tooth with minimal remaining structure.
How to prepare
- Bring earlier X-rays and root canal records, and expect a CBCT scan.
- Tell your dentist about allergies, medicines and any heart valve conditions; do not stop any medicine yourself.
- Take any antibiotics or painkillers exactly as prescribed.
- Eat a normal meal beforehand and arrange soft foods afterwards.
- Ask about the restoration the tooth will need so you can plan visits.
Questions to ask your surgeon
- Why did the first root canal fail, and can it be fixed?
- Will you use a microscope and a 3D scan?
- Is surgery such as apicoectomy a better option?
- How many visits do I need?
- Will I need a new crown?
- Who can follow up at home?
Red flags when comparing hospitals
- No new X-rays or 3D scan before retreatment.
- No magnification or rubber dam isolation.
- Not discussing extraction and implant as alternatives.
- No plan for restoring the tooth afterwards.
