About Cystoscopy + Biopsy

A flexible or rigid cystoscope is passed into the bladder, which is filled with fluid to give a clear view. If the surgeon sees a suspicious area, small forceps or a cutting loop remove a piece of tissue for the pathologist. Flexible cystoscopy can be done awake with local gel, while biopsies are often taken under spinal or general anaesthesia.

The procedure gives a clear answer about what is in the bladder and whether abnormal tissue is cancer, inflammation or another cause. Accuracy depends on a thorough inspection of every part of the bladder, the number and depth of biopsies and the pathologist's expertise. Burning on passing urine and pink urine for a few days are common.

This is usually a day case, and most people can fly after one or two days when they are passing urine comfortably. Plan to wait several days for the pathology report, ideally while you are still near the hospital, or arrange for it to be sent securely to your doctor. Drink plenty of water and report fever or clots.

Usually a good fit if

  • Visible or microscopic blood in the urine without a clear cause, particularly in smokers or older adults.
  • A suspicious area seen on a scan or earlier cystoscopy that needs tissue diagnosis.
  • Follow-up of known bladder tumours or unexplained persistent bladder symptoms.
  • Fit for the planned form of anaesthesia.

May need to wait if

  • An active urinary tract infection should be treated before the procedure.
  • Blood-thinning medicines may need a plan from your doctor before biopsy.
  • A severe urethral stricture may need a different approach or dilatation.
  • Medical conditions that make spinal or general anaesthesia risky need optimising.
How to prepare
  1. Bring any CT, ultrasound, urine cytology and earlier cystoscopy reports so the surgeon sees the full picture.
  2. Have a urine test before the procedure, since infection must be treated first.
  3. Tell your doctor about blood thinners and aspirin; stopping them is only on the doctor's instruction.
  4. Follow fasting instructions if you are having spinal or general anaesthesia, and arrange a lift home.
  5. Ask when pathology results will be ready, so you can plan your stay.
Questions to ask your surgeon
  1. Why do I need a biopsy, and what are you looking for?
  2. Will it be done with a flexible or rigid scope, and under which anaesthesia?
  3. If you find a tumour, can you remove it in the same procedure?
  4. When will I get pathology results, and who will explain them?
  5. What bleeding or infection should I watch for, and when should I seek help?
  6. Who can arrange follow-up cystoscopies at home?
Red flags when comparing hospitals
  • Biopsy is proposed with no clear reason, scan or history to justify it.
  • No urine test or infection treatment beforehand.
  • Pathology is not reviewed by a qualified pathologist or results are not provided in writing.
  • No plan if a tumour is found or if bleeding continues.
