About Bilateral Total Hip Replacement
Bilateral total hip replacement replaces both hip joints with implants. It is for people with arthritis or damage in both hips who have pain and limited walking.
In each hip the surgeon removes the damaged ball of the thigh bone and the worn socket surface, and fits a new ball and socket of metal, ceramic or plastic. Spinal or general anaesthesia may be used. Both hips can be done in one session or a few weeks to months apart. Staging is often chosen when health is a concern.
Usually a good fit if
- Severe arthritis, avascular necrosis or damage in both hips with pain and loss of function.
- Fit enough for a larger operation if done together.
- Failed non-surgical treatments.
- Motivated for rehabilitation.
May need to wait if
- Heart, lung or kidney disease that makes simultaneous surgery risky.
- Active infection or poorly controlled diabetes.
- Frailty, where staged surgery is safer.
- Severe bone loss or deformity needing special implants.
How to prepare
- Have X-rays of both hips and the pelvis, plus heart and lung assessments if simultaneous surgery is planned.
- Review all medicines, including blood thinners, with your doctor; do not change them yourself.
- Treat dental problems and skin infections, and control diabetes and blood pressure before surgery.
- Prepare your home with raised seats and rails, and arrange help.
- Ask about clot prevention and fitness to fly.
Questions to ask your surgeon
- Should both hips be done together or in stages?
- Which approach and implants will you use and why?
- How many bilateral hip replacements do you do and what are the results?
- What clot and infection prevention will I have?
- What movement precautions apply?
- What physiotherapy can I do at home?
Red flags when comparing hospitals
- Offering simultaneous surgery without heart and lung assessment.
- No clot prevention plan.
- No discussion of staging.
- No rehabilitation or follow-up plan.
