About BCG Bladder Instillation Course
After the tumour has been removed through the urethra, a thin catheter fills the bladder with BCG solution, which is held for about two hours before being passed. An induction course is usually given once a week for six weeks, and many patients continue with maintenance doses over one to three years. Your urologist chooses the schedule based on risk.
BCG lowers the risk of recurrence and progression in many patients, especially those with higher-risk tumours. Outcomes depend on the grade and stage of the original cancer, how completely it was removed, and whether the full schedule is completed. Common side effects are bladder irritation and flu-like symptoms for a day or two, and serious infection is rare.
The course runs over weeks to years, so most people arrange the induction in one place or share the follow-up with a local urologist who can give maintenance doses. Check beforehand that BCG supply and cystoscopy are available at home. Bring your pathology report and operation notes. Report fever or blood clots promptly.
Usually a good fit if
- Non-muscle-invasive bladder cancer, such as carcinoma in situ or high-grade papillary tumours, after complete resection.
- Intermediate or high-risk disease where lowering recurrence matters.
- A well-managed bladder that can hold the solution and pass urine normally.
- Fit enough to attend the weekly sessions.
May need to wait if
- Active urinary infection, visible blood in the urine from a recent resection or a traumatic catheterisation means treatment must wait.
- A weakened immune system, active tuberculosis or previous serious BCG reaction is a reason to reconsider.
- Muscle-invasive or metastatic bladder cancer needs a different plan.
- Pregnancy or breastfeeding needs specialist advice.
How to prepare
- Have a urine test before each visit, as active infection means treatment must wait; bring recent urine and pathology reports.
- Avoid drinking much fluid for several hours before so the BCG stays in contact with the bladder lining.
- Tell your doctor about immune problems, steroids or other immunosuppressive drugs; any medicine change is only on the doctor's instruction.
- Discuss safe handling: pass urine sitting down and add bleach to the toilet for six hours after treatment.
- Plan the weekly schedule so you can stay near the centre for the full six-week induction.
Questions to ask your surgeon
- Is my tumour risk level one for which BCG is recommended, and for how long?
- How many weekly instillations and maintenance doses are planned, and how will I tolerate them?
- What side effects should I expect, and when should I stop treatment and call you?
- Where will cystoscopy checks be done during and after the course?
- What BCG strain and supply does your centre use, and are shortages a concern?
- Can my home urologist give the remaining doses if I travel?
Red flags when comparing hospitals
- BCG is offered without the pathology report or a complete tumour resection beforehand.
- Instillations are given within days of surgery, or through a traumatic catheterisation, risking BCG spreading into the blood.
- No guidance about handling urine or recognising a serious reaction.
- No plan for check cystoscopies after the course.
Questions about BCG Bladder Instillation Course abroad
Which country is cheapest for BCG Bladder Instillation Course?
The lowest typical hospital prices are in India ($2,553), Thailand ($3,008), Turkey ($3,583), out of 9 countries we compare.
What side effects are normal?
Bladder irritation, frequent urination, blood in the urine and mild flu-like symptoms for a day or two are common. High fever, chills or worsening illness need urgent medical help.
Is BCG the same as the TB vaccine?
It uses the same weakened bacterium, but here it acts on the bladder lining to stimulate a local immune response. It is not given as a vaccine.
Will it cure my cancer?
BCG reduces the chance of recurrence and progression, but does not guarantee a cure. Regular cystoscopies remain essential. Your urologist can discuss your risk.
Can I have sex during treatment?
Doctors often advise condoms for a week or so after each dose to protect partners. Ask your treating team.
