About Anti-VEGF Injection (Lucentis / Eylea)

This is an outpatient treatment with numbing drops and no surgery. The eye and lids are cleaned with antiseptic, a lid holder is placed, and the medicine is injected through the white of the eye with a very fine needle. The visit takes about 15 minutes. Different drugs and biosimilars are available, and the choice depends on your condition and clinician.

The injections usually stabilise vision and in some people improve it, but they treat the disease rather than cure it. Most people need a series of injections, often monthly at first, then spaced out according to scans. Results depend on how early treatment starts, the underlying condition and sticking to the schedule.

Because treatment is ongoing, a short trip rarely covers a full course. Plan for an initial loading phase of injections, usually three monthly doses, and make sure a retina clinic at home can continue. Bring OCT scans and your medical history, and see a doctor urgently if you develop pain, worsening redness or falling vision.

Usually a good fit if

  • Wet age-related macular degeneration.
  • Diabetic macular oedema or retinal vein occlusion causing swelling at the macula.
  • Myopic choroidal neovascularisation or other leaking retinal vessels, as advised.
  • Able to attend regular injections and scans.

May need to wait if

  • Active eye or eyelid infection, which must clear first.
  • Allergy to the medicine or antiseptic.
  • Recent stroke or heart attack, where your doctor should weigh the risks.
  • Pregnancy, where the use should be discussed carefully.
How to prepare
  1. Bring your latest macular scan (OCT), fundus photos and diabetes or blood pressure records.
  2. Have your glucose and blood pressure controlled; discuss any blood thinners with your doctor and do not stop them yourself.
  3. Tell the team about allergies to iodine or antiseptic and any recent eye infection.
  4. Arrange transport, as vision may be blurred for a few hours.
  5. Ask how you will receive repeat injections, as the course is usually a series.
Questions to ask your surgeon
  1. Which drug do you recommend for my condition, and why?
  2. How many injections should I expect in the first year?
  3. What scan and vision results will show it is working?
  4. Will you use a treat-and-extend or fixed schedule?
  5. How can I get follow-up injections at home, with the same drug?
  6. What symptoms after injection need an urgent call?
Red flags when comparing hospitals
  • No OCT scan to decide treatment.
  • Offering one injection as a permanent fix.
  • No clear plan for continuing the series at home.
  • Reusing vials or poor sterile technique.
