About Allogeneic Bone Marrow Transplant
An allogeneic bone marrow transplant uses stem cells from a matched donor to replace diseased bone marrow and build a new immune system. It is used for leukaemia, lymphoma, aplastic anaemia and some inherited blood disorders.
After conditioning with chemotherapy, sometimes with radiation, to clear the diseased marrow, donor stem cells are given through a vein like a transfusion. Donors may be a sibling, an unrelated registry match or a partly matched (haploidentical) relative. Over the following weeks the cells settle in the marrow and begin making new blood cells.
Usually a good fit if
- High-risk or relapsed acute leukaemia, myelodysplastic syndrome or other blood cancers that need a graft-versus-leukaemia effect.
- Severe aplastic anaemia or inherited marrow failure.
- Thalassaemia or sickle cell disease in selected patients with a matched donor.
- A suitable HLA-matched sibling, unrelated or haploidentical donor, and adequate fitness.
May need to wait if
- No suitable donor identified.
- Severe heart, lung, liver or kidney impairment limiting the ability to withstand conditioning.
- Active uncontrolled infection.
- Disease status or fitness where risks outweigh benefit, to be judged by the transplant team.
How to prepare
- Bring your diagnosis reports, bone marrow results and treatment history, and the donor's HLA typing if already known.
- Do not stop any medicine unless your doctor instructs you, and review all supplements with the team.
- Expect extensive tests of heart, lung, kidney, liver and infection screening for you and the donor.
- Plan for a central line, dental clearance and vaccination review.
- Arrange a carer for the long stay and a place to live nearby after discharge.
Questions to ask your surgeon
- Why is allogeneic transplant needed rather than chemotherapy or an autologous transplant?
- How well matched is the donor, and what is the type of transplant?
- What conditioning regimen will I receive, and what are the risks?
- How will graft-versus-host disease be prevented and treated?
- How many allogeneic transplants does your centre do each year, and what are the one-year outcomes for my disease?
- What follow-up do I need at home?
Red flags when comparing hospitals
- No dedicated transplant unit with isolation rooms.
- No HLA typing or donor search plan.
- Unwillingness to share the centre's own outcomes.
- No long-term follow-up or graft-versus-host management plan.
