À propos de Résection hépatique (tumeurs bénignes)

The operation is performed under general anaesthesia and may be open, laparoscopic or robotic, depending on the size and position of the lesion. The surgeon maps the liver with ultrasound, divides the liver tissue around the lesion with energy devices and controls blood vessels and bile ducts. The liver is able to regrow, so a healthy remnant is left behind.

Removal usually cures the problem and relieves pain or pressure, and the liver regenerates over a few weeks to months. Outcomes depend on the amount of liver is removed, the health of the rest of the liver, the lesion's location and the team's experience. Possible complications include bleeding, bile leakage and temporary liver dysfunction.

Expect about a week in hospital and plan to stay nearby for around three weeks, with liver blood tests and a scan before flying. Bring MRI or CT reports and any biopsy results. Avoid heavy lifting and alcohol during healing. Share the pathology report with your doctor at home to confirm the tumour was benign and plan any follow-up.

Généralement adapté si

  • Hepatic adenoma that is large (about 5 cm or more), growing or bleeding.
  • Symptomatic haemangioma or focal lesion.
  • Benign lesion that cannot be distinguished from cancer.
  • Complications such as rupture or pressure on neighbouring organs.

Il peut falloir attendre si

  • Small asymptomatic lesions, such as most haemangiomas or focal nodular hyperplasia, which can be watched.
  • Poor liver function or cirrhosis limiting the safe amount that can be removed.
  • Severe heart or lung disease unfit for major surgery.
  • Uncorrected clotting disorder.
Comment se préparer
  1. Triple-phase CT or MRI defines the tumour and liver anatomy, and may measure what volume of liver would remain.
  2. Blood tests including liver function, clotting and, if relevant, alpha-fetoprotein.
  3. Review hormonal contraceptives, blood thinners and other medicines with your doctor; do not stop them yourself.
  4. Stop alcohol and optimise nutrition and diabetes control.
  5. Anaesthetic assessment and fasting instructions.
Questions à poser à votre chirurgien
  1. Is surgery really needed, or can this lesion be watched?
  2. What proportion of the liver will be removed and what will remain?
  3. Will surgery be open or minimally invasive?
  4. What are the risks of bleeding and bile leak?
  5. How many liver resections do you perform each year?
  6. What scans and blood tests will I need at home?
Signaux d'alerte à surveiller en comparant les hôpitaux
  • Surgery for a small lesion with no discussion of surveillance.
  • No high-quality imaging beforehand.
  • No liver-volume planning.
  • Centre without intensive care or blood bank support.
  • No explanation of risks.
