Esophageal Stricture (Benign)

A benign oesophageal stricture is a narrowing of the gullet caused by scar tissue rather than cancer. Long-standing acid reflux is the most common cause, and it makes swallowing solid food difficult.

À propos de esophageal stricture (benign)

Ce qu'il faut savoir

People notice food sticking after swallowing, chest discomfort, regurgitation and sometimes weight loss. Other causes include swallowing caustic substances, radiotherapy, eosinophilic oesophagitis and previous surgery. Diagnosis is by endoscopy, with biopsies to exclude cancer, and a barium swallow may map the narrowing. Without treatment, strictures tend to tighten gradually and can recur.

First-line treatment is endoscopic dilation, where the narrowed segment is stretched with a balloon or bougie, often repeated over several sessions. Acid-suppressing medicine such as a proton pump inhibitor is given to reduce relapse, and steroid injection or a temporary stent may be added for stubborn strictures. Surgery is reserved for strictures that keep returning. Treatment abroad is rarely needed unless care is complex.

Bilan avant le départ

Examens à déjà réalisés

  • Your gastroscopy report and photographs showing the length, position and tightness of the narrowing, plus any biopsy results confirming it is not cancer.
  • A barium swallow or CT report if one was done, to show the stricture's shape and any associated hiatus hernia.
  • Records of previous dilatations: dates, balloon or bougie sizes reached, complications and how long you stayed symptom-free.
  • The medicines you take for reflux, including acid-suppressing tablets and how long you have used them.
  • Your weight history and recent blood tests for anaemia and nutrition, since swallowing difficulty often causes weight loss.

Deuxième avis

Quand obtenir un second regard

  • Dilatation has been needed several times within a few months and nobody has discussed what is driving the recurrence.
  • The stricture followed swallowing a caustic substance and you have been offered only repeated stretching.
  • Biopsies were not taken from the narrowed area, or you are unsure whether cancer has been excluded.
  • A stent, bypass or removal of the oesophagus has been suggested for a stricture that has not been treated otherwise.

Questions à poser

  1. What caused my stricture, and is the underlying cause being treated so it does not keep returning?
  2. Was the narrowing biopsied, and has cancer been ruled out?
  3. How many dilatations are usually needed, and what are the risks such as perforation?
  4. Would steroid injection, a temporary stent or surgery be considered if dilatation fails?
  5. What should I eat and how should I take tablets while the narrowing is present?
  6. Do I need long-term acid suppression or surveillance endoscopy?

Perspectives

Pronostic réaliste

Most benign oesophageal strictures caused by reflux respond well to endoscopic dilatation together with strong acid suppression, and swallowing usually improves quickly. Some strictures return and need repeat dilatation, particularly if reflux is not controlled or the scar is long, tight or caused by caustic injury or radiotherapy. Perforation is uncommon but is the main procedural risk. Refractory cases may need steroid injection, temporary stenting or, rarely, surgery. Long-term follow-up matters because the underlying cause continues to influence recurrence.

Bilan diagnostique

04 · FAQ

Questions fréquentes sur esophageal stricture (benign)

Will I need repeated dilatations?

Some people do. Many strictures need more than one session, especially if acid reflux is not well controlled. Treating the cause alongside stretching lowers the chance of recurrence.

Is a benign stricture the same as cancer?

No, but a narrowing can look similar on a scan, so biopsies are usually taken from the area to confirm it is benign.

Can I fly with a narrowed gullet?

Flying is generally not a problem in itself, but you should be able to swallow fluids and saliva comfortably, and a recent dilatation needs your doctor's clearance first.

How is the procedure done?

Dilatation is usually done during a gastroscopy under sedation, using balloons or graduated dilators to widen the narrowing. Most people go home the same day.

Clinical context

Working up esophageal stricture (benign) properly

A condition like esophageal stricture (benign) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a chirurgie digestive specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or chirurgie digestive specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
