About Wisdom Tooth Extraction

Wisdom tooth extraction removes one or more of the back molars that come through late in the teens or twenties. It is recommended when they are impacted, decayed, infected or crowding other teeth.

The procedure is usually done under local anaesthesia, with sedation or general anaesthesia for anxious patients or complex cases. A small cut is made in the gum, and bone around the tooth may be trimmed. The tooth is often divided into sections to be removed, and the area is stitched. A scan or panoramic X-ray beforehand shows how close the roots are to the nerve in the lower jaw.

Usually a good fit if

  • Impacted wisdom teeth causing repeated infection, pain or swelling.
  • Decay or damage to a wisdom tooth or its neighbour that cannot be restored.
  • Cysts or gum disease around the tooth.
  • Teeth that are advised for removal before orthodontics or other dental treatment.

May need to wait if

  • Fully erupted, healthy and cleanable wisdom teeth with no symptoms.
  • Bleeding disorders or blood thinners not yet reviewed by your doctor.
  • Recent head and neck radiotherapy or bisphosphonate treatment needing specialist planning.
  • An acute spreading infection with fever, which may need treatment first.
How to prepare
  1. Have a panoramic X-ray or 3D scan so the surgeon can assess nerve position and tooth angle.
  2. Tell the surgeon about blood thinners, other medicines and allergies; any change is on your doctor's instruction.
  3. If sedation or general anaesthesia is planned, follow the fasting instructions given.
  4. Bring someone with you, stock soft food and cold packs, and ask when it is safe to fly.
Questions to ask your surgeon
  1. Do all my wisdom teeth need to come out, or only the symptomatic ones?
  2. How close is the nerve on my scan, and what are the risks?
  3. Which anaesthesia do you recommend and who will give it?
  4. What is the usual recovery, and when can I eat, work and fly?
  5. What happens if I get a dry socket or infection after returning home?
Red flags when comparing hospitals
  • Teeth are removed without an X-ray or scan.
  • All four are advised automatically without a clear reason.
  • The surgeon cannot discuss nerve injury or dry socket risk.
  • No aftercare plan or emergency contact is provided.
