About TURBT (Transurethral Resection of Bladder Tumour)

TURBT removes bladder tumours through the urethra using a camera instrument, with no external cut. It confirms the diagnosis, shows how deep the tumour goes, and can treat early non-invasive bladder cancer.

Under spinal or general anaesthesia, a rigid cystoscope is passed through the urethra and the bladder is filled with fluid. The surgeon removes the tumour with an electrical loop, either in pieces or as a single block (en bloc), and takes samples including muscle to judge depth. Some units use blue-light or narrow-band imaging to spot flat tumours.

Usually a good fit if

  • Visible bladder tumour on cystoscopy or imaging.
  • Blood in the urine that needs diagnosis.
  • Suspected non-muscle-invasive bladder cancer that can be removed endoscopically.
  • Fit for spinal or general anaesthesia.

May need to wait if

  • Active urinary infection that needs treatment first.
  • Bleeding disorder or blood thinners not yet managed by your doctor.
  • Urethral narrowing that prevents instrument passage, which may require preliminary treatment.
  • Tumour already known to be very advanced, where a different plan may be better.
How to prepare
  1. Bring cystoscopy findings, urine cytology and CT urogram or ultrasound so the team knows the tumour size and number.
  2. Review blood thinners and aspirin with your doctor; do not stop them yourself.
  3. Have a urine culture; infection must be treated before surgery.
  4. Fast as instructed for spinal or general anaesthesia.
  5. Plan to stay close for the first days, as bleeding or catheter issues can occur.
Questions to ask your surgeon
  1. Will the sample include bladder muscle, and will a second look be needed?
  2. Will a single chemotherapy dose be put in the bladder right after surgery?
  3. Do you use blue light or narrow-band imaging to find small tumours?
  4. How will the grade and stage guide further treatment such as BCG?
  5. How often will check cystoscopies be needed, and can they be done near home?
  6. What bleeding or retention signs need urgent contact?
Red flags when comparing hospitals
  • Tumour removed without any bladder muscle sampled when deeper disease is possible.
  • No pathology review with stage and grade.
  • No plan for follow-up cystoscopy schedule.
  • No catheter or bleeding care plan given.
