About Trabeculectomy (Glaucoma)

Trabeculectomy creates a new drainage channel under the conjunctiva to lower eye pressure. It is a long-standing operation for glaucoma that is not controlled with drops or laser, aiming to prevent further optic nerve damage.

Trabeculectomy is done under local anaesthesia with sedation. The surgeon lifts the conjunctiva, creates a partial-thickness trapdoor flap in the white of the eye, removes a small piece of tissue beneath it and closes the flap with stitches. Fluid then drains into a raised pocket (a bleb) under the conjunctiva. Mitomycin C is often applied to limit scarring.

Usually a good fit if

  • Glaucoma progressing despite maximum drops and laser.
  • Need for a low target pressure to protect the optic nerve.
  • Healthy, mobile conjunctiva.
  • Able to attend frequent post-operative visits.

May need to wait if

  • Neovascular glaucoma or heavily scarred conjunctiva, where a tube may suit better.
  • Active eye infection or inflammation.
  • Inability to attend frequent visits in the first weeks.
  • Only eye with good vision, where risk needs particularly careful discussion.
How to prepare
  1. Bring visual field tests, optic nerve scans and pressure records, and details of earlier eye surgery.
  2. Review blood thinners and glaucoma drops with your doctor; do not stop them yourself.
  3. Plan close follow-up for several weeks after surgery near the hospital.
  4. Tell the surgeon about previous eye inflammation or conjunctival scarring.
  5. Arrange transport and a carer for the first days.
Questions to ask your surgeon
  1. Is trabeculectomy or a tube/MIGS better for my glaucoma?
  2. Will mitomycin C be used and what are its risks?
  3. What pressure target are you aiming for?
  4. What frequent post-operative adjustments should I expect?
  5. What are the signs of a leaking bleb or infection?
  6. Who can monitor the bleb at home?
Red flags when comparing hospitals
  • No visual field or optic nerve assessment before surgery.
  • No plan for frequent post-operative checks.
  • Not explaining the bleb or the risks of leaks and infection.
  • Promising to restore lost vision.
