About Total Knee Replacement

Total knee replacement removes worn joint surfaces in the knee and replaces them with metal and plastic implants. It is for adults whose arthritis causes pain and limits walking despite non-surgical treatment.

The surgeon removes thin layers of damaged bone and cartilage from the thigh bone, shin bone and often the kneecap, then fits implants shaped to match, held with bone cement or press-fit. Surgery takes around one to two hours, with general or spinal anaesthesia. Implant design, alignment technique and computer or robotic guidance vary between surgeons.

Usually a good fit if

  • Advanced knee arthritis with pain on walking, at rest or at night, seen on X-ray.
  • Daily activities limited despite physiotherapy, medicines and injections.
  • Knee deformity or stiffness affecting independence.
  • General health suitable for anaesthesia and rehabilitation.

May need to wait if

  • Active infection in the knee or anywhere else in the body.
  • Severe uncontrolled medical conditions such as unstable heart disease or poorly controlled diabetes.
  • Poor leg circulation or skin that will not heal.
  • Mild arthritis that may still respond to non-surgical care.
How to prepare
  1. Bring knee X-rays and a list of your medicines; the surgeon will review the extent of wear.
  2. Ask your doctor about blood thinners, anti-inflammatories and diabetes medicines to review before surgery.
  3. Complete blood tests, ECG and an anaesthetic review; treat dental, skin or urinary infection first.
  4. Lose weight and strengthen thigh muscles with exercises if your surgeon recommends.
  5. Arrange crutches, home help and a safe time to fly home.
Questions to ask your surgeon
  1. Do I need a total or partial knee replacement?
  2. Which implant and technique will you use, and what is its track record?
  3. Will you use a tourniquet, drain or nerve block?
  4. How soon will I walk, bend my knee, drive and fly?
  5. How many knee replacements do you do each year, and what are your infection and revision rates?
  6. What physiotherapy and X-ray follow-up can be arranged back home?
Red flags when comparing hospitals
  • Surgery recommended without X-rays or a trial of non-surgical care.
  • No details of the implant brand or model offered in writing.
  • No clot-prevention or early walking plan.
  • Pressure to book surgery before a full medical assessment.
