★★★★ Liver transplant at Apollo — long but worth it
Getting the living-donor workup paperwork together took months. Once in Delhi, the team moved fast. Year-one immunosuppression protocol is working. I wish I'd started with them earlier instead of six months in São Paulo.

About Split Liver Transplant

The donor liver is carefully divided into two working portions, typically a smaller left section for a child and a larger right section for an adult, each with its own blood supply and bile duct. The split can be done in the donor's body or on the bench. Each recipient then has a standard liver transplant under general anaesthesia.

Results can be close to whole-organ transplants in carefully chosen recipients, though complications such as bile leaks or blood-vessel problems are somewhat more likely. Outcomes depend on how ill you are, the size match between graft and recipient, and the surgical team's experience with this technique. Lifelong anti-rejection medicines are needed.

Expect around three weeks in hospital, followed by months of close review near the centre. The timing depends on a suitable donor and matched recipient pair being found. Bring full records, ask whether your size and condition suit a split graft, and arrange follow-up and medicine supply at home.

Usually a good fit if

  • Children with biliary atresia, metabolic liver disease or acute failure, who can receive the left lateral segment.
  • Adults of suitable size with end-stage liver disease and a stable condition for a right lobe graft.
  • Patients on a deceased donor list at a centre experienced in splitting.
  • Fit enough for major surgery.

May need to wait if

  • Very sick adults who need a full-size liver, where a partial graft may be too small.
  • Cancer beyond transplant criteria or uncontrolled infection.
  • Severe heart, lung or kidney disease.
  • No carer or inability to take lifelong medicines.
How to prepare
  1. Complete the standard transplant assessment: liver function, imaging, heart and lung tests, infection screening and cancer checks.
  2. Ask whether your size and condition suit a partial graft and how the centre allocates split grafts.
  3. Report any bleeding, confusion or fluid build-up to your liver doctor at once.
  4. Review all medicines with your doctor; do not stop anything yourself.
  5. Plan several weeks near the hospital with a carer, and a fitness-to-fly review.
Questions to ask your surgeon
  1. How many split liver transplants do you do, and what are your results for both recipients?
  2. Is the graft split in the donor or on the back table?
  3. Will I receive the larger or smaller graft, and is it big enough for me?
  4. How do you manage bile leaks, which are more common with split grafts?
  5. What are my graft survival and one-year survival figures?
  6. How will care be shared with my local doctor?
Red flags when comparing hospitals
  • No experience in splitting livers or no published results.
  • Offering split grafts to patients whose size does not fit, with no discussion of risks.
  • Any suggestion that organs can be bought.
  • No plan for bile duct and vessel complications.

Questions about Split Liver Transplant abroad

Which country is cheapest for Split Liver Transplant?

The lowest typical hospital prices are in India ($70,210), Thailand ($83,160), Turkey ($95,895), out of 9 countries we compare.

How long will I stay in hospital for Split Liver Transplant?

Most patients spend about 21 nights in hospital. Full recovery takes about 120 days; your surgeon will tell you when you are fit to fly home.

How is it different from a whole liver transplant?

The liver is divided so two patients benefit. Bile duct leaks and vessel problems are somewhat more common, so centre experience matters.

Who gets which part?

Usually the left lateral segment goes to a child and the larger right lobe to an adult.

Will the partial liver grow?

Yes. A partial graft regenerates to match the recipient's size within weeks to months.

Are results as good as whole-liver transplants?

In experienced centres, results are comparable, though complications can be higher.
