About Simultaneous Pancreas-Kidney (SPK) Transplant

Both organs normally come from the same deceased donor and are placed in the abdomen in one operation under general anaesthesia. The kidney is joined to blood vessels and the bladder, and the pancreas to blood vessels and the bowel or bladder. You are assessed for heart disease and circulation before listing, as diabetes can affect both.

A successful transplant can end dialysis and the need for insulin injections, and improves blood sugar control. Results depend on your cardiovascular health, the quality of the organs, rejection, and surgical complications such as clots or leaks. Lifelong anti-rejection medicines are required, and the kidney often does better than the pancreas over the long term.

Expect about three weeks in hospital, then regular tests and clinic visits during the first months, so plan to stay near the team. Bring diabetes, dialysis and heart records. Check how organ allocation works, and arrange how your local doctors will take over medicines and monitoring afterwards.

Usually a good fit if

  • Type 1 diabetes with kidney failure or advanced kidney disease needing dialysis.
  • Fit enough for combined surgery, with acceptable heart health.
  • Unstable diabetes or hypoglycaemia unawareness together with kidney disease.
  • Able to take lifelong anti-rejection tablets.

May need to wait if

  • Severe heart disease or poor pelvic vessels.
  • Active infection or recent cancer.
  • Obesity or type 2 diabetes with high insulin resistance, depending on the centre's policy.
  • Poor adherence to treatment or no carer.
How to prepare
  1. Complete the assessment: heart tests, vessel imaging, infection screening and cancer checks, with careful review of diabetic complications.
  2. Record insulin doses and glucose patterns; do not change insulin without your doctor's instruction.
  3. Keep dialysis on schedule and review medicines with your transplant team.
  4. Confirm the centre performs SPK transplants regularly and accepts international patients.
  5. Plan several weeks near the hospital with a carer.
Questions to ask your surgeon
  1. How many SPK transplants do you do each year, and what are your one-year graft survival results?
  2. Is the pancreas drained to the bowel, and why?
  3. What is the chance I will come off insulin completely?
  4. How do you prevent graft clots and pancreatitis?
  5. Would a kidney transplant alone be better for me?
  6. How will sugar, enzymes and kidney function be monitored at home?
Red flags when comparing hospitals
  • No published SPK volumes or graft survival results.
  • Promising insulin freedom without explaining the risks.
  • Any suggestion that organs can be bought.
  • No plan for monitoring pancreas enzymes, glucose and kidney function.

Questions about Simultaneous Pancreas-Kidney (SPK) Transplant abroad

Which country is cheapest for Simultaneous Pancreas-Kidney (SPK) Transplant?

The lowest typical hospital prices are in India ($70,470), Thailand ($83,258), Turkey ($103,123), out of 9 countries we compare.

How long will I stay in hospital for Simultaneous Pancreas-Kidney (SPK) Transplant?

Most patients spend about 21 nights in hospital. Full recovery takes about 120 days; your surgeon will tell you when you are fit to fly home.

Will I be free of dialysis and insulin?

Many patients are. Both can return if either organ fails, and the kidney tends to work more reliably than the pancreas.

What are the main risks?

Clotting of the pancreas vessels, bleeding, leaks, rejection, infection and the effects of immunosuppression.

Why combine the two organs?

Diabetes damages the kidneys. One operation treats both, and the new kidney is protected from further diabetic damage.

How long do the grafts last?

Many last five to ten years or longer, and the kidney often lasts longest.
