About Single Lung Transplant

Under general anaesthesia, the surgeon removes the diseased lung through a chest incision and attaches the donor lung to the airway, pulmonary artery and veins. A heart-lung machine may be used if needed. It is shorter than a double lung operation, and one donor can sometimes provide lungs for two recipients.

Breathing and exercise ability usually improve, though less than with two lungs in some conditions. Results depend on the underlying disease, your age and fitness, how the transplanted and remaining lungs work together, infection and rejection. Chronic rejection of the lung limits long-term survival, so lifelong immunosuppression and monitoring are essential.

Plan for about three weeks in hospital and then rehabilitation and frequent clinic visits near the transplant team for several months. You will need someone with you. Ask how listing and organ offers work, and arrange for your own doctors to manage lung-function testing and medicines at home.

Usually a good fit if

  • End-stage pulmonary fibrosis or emphysema without chronic infection in the lungs.
  • Declining despite maximal medical treatment and oxygen.
  • Strong enough for major surgery without other serious organ failure.
  • Able to follow lifelong medication and monitoring.

May need to wait if

  • Suppurative lung disease such as cystic fibrosis or bronchiectasis, where one lung would leave infection behind.
  • Active cancer, active infection outside the lungs or major heart, liver or kidney disease.
  • Current smoking or substance use, or poor adherence.
  • Severe pulmonary hypertension with heart strain, where a double lung transplant is preferred.
How to prepare
  1. Complete the transplant assessment: lung function, exercise tests, heart catheterisation, scans, infection screening and cancer checks.
  2. Join pulmonary rehabilitation on your doctor's advice to keep strength.
  3. Review all medicines, including oxygen and steroids, with your doctor; do not change them yourself.
  4. Ask about listing in the destination country and how long you must stay nearby.
  5. Arrange a fitness-to-fly review and medical oxygen for travel.
Questions to ask your surgeon
  1. Do you recommend a single or double lung transplant for me, and why?
  2. How many lung transplants do you do each year, and what are the results?
  3. Does your programme accept international patients, and what is the likely wait?
  4. How do you monitor rejection and infection after discharge?
  5. What happens to my remaining diseased lung over time?
  6. Which drugs can my local doctor monitor?
Red flags when comparing hospitals
  • No published lung transplant volumes or survival results.
  • Promising a lung on a fixed date without explaining allocation.
  • Any suggestion that organs can be bought.
  • No plan for spirometry, bronchoscopy and rejection monitoring.
