About Sigmoid Colectomy
Sigmoid colectomy removes the sigmoid colon, the S-shaped section of large bowel before the rectum. It is used for complicated or recurrent diverticular disease, tumours and some other bowel problems.
Through an open abdominal incision the surgeon removes the diseased sigmoid segment and joins the remaining ends of the bowel together. For some people, particularly after an emergency or in cancer, a temporary stoma may be made. Keyhole surgery is an option in many cases, but this open approach is chosen for complex or scarred bowel.
Usually a good fit if
- Recurrent or complicated diverticulitis with abscess, fistula, stricture or repeated hospital admissions.
- A cancer or large polyp in the sigmoid colon that cannot be removed endoscopically.
- Sigmoid volvulus (twisted bowel) that keeps recurring or has damaged the bowel.
- Fitness for major abdominal surgery, with heart and lung function assessed beforehand.
May need to wait if
- A single, mild diverticulitis attack that settled with antibiotics, which usually does not need surgery.
- Active infection or abscess that needs to be drained or treated first.
- Serious heart, lung or kidney disease that has not been optimised for anaesthesia.
- Unexplained bowel symptoms with no colonoscopy yet done to exclude cancer.
How to prepare
- Have a colonoscopy (or CT colonography) and a CT scan so the surgeon can see the disease, and bring reports and biopsies.
- Ask your doctor about blood thinners, diabetes medicines and steroids, and follow only the instructions you are given.
- Follow bowel preparation instructions, which may include oral laxatives and antibiotics, as set by the surgical team.
- Have blood tests for anaemia and kidney function, and improve nutrition if you have lost weight.
- Ask the team when you can fly home, since long flights soon after abdominal surgery raise the clot risk.
Questions to ask your surgeon
- Is an open or keyhole operation right for me, and what is the chance you will need to convert?
- Will I need a stoma, and if so, how long might it last?
- How many colon resections do you perform every year, and how do you reduce the risk of a leak at the join?
- Is enhanced recovery used, with early feeding and walking?
- What will the pathology report tell us, and what happens if cancer is found?
- What follow-up tests do I need at home, and who reviews the results?
Red flags when comparing hospitals
- Surgery planned without a colonoscopy or CT scan to confirm the diagnosis.
- No discussion of the risk of a leak at the join or of a possible temporary stoma.
- No enhanced-recovery plan or no intensive-care backup for complications.
- Pressure to operate for a single mild attack of diverticulitis without a specialist opinion.

They may be irregular at first, with looser or more frequent stools, but usually settle over weeks. A fibre-rich diet and fluids help.

A leak is when the join between the bowel ends breaks down. It is uncommon but serious and shows as fever, severe pain or a fast heart rate. You would need urgent care.
Is surgery needed for diverticulitis?
Not always. It is advised for repeated or complicated attacks, and each case is individual. Ask your surgeon why it is advised for you.
