About Pyeloplasty
Pyeloplasty is surgery to widen a blocked junction between the kidney and the ureter. It restores free drainage of urine and prevents further kidney damage, and is used for adults and children.
The surgeon removes the narrowed section of the pelvi-ureteric junction and rejoins the healthy ureter to the kidney pelvis, or uses another reconstructive technique when the anatomy is unusual. It can be performed open, laparoscopically or with robotic assistance under general anaesthesia. A ureteric stent usually remains for four to six weeks to protect the join while it heals.
Usually a good fit if
- Symptomatic pelvi-ureteric junction obstruction with flank pain, stones or repeated infections.
- Reduced or falling kidney function on a renogram in the affected kidney.
- Progressive swelling of the kidney (hydronephrosis) on follow-up scans.
- A patient fit for general anaesthesia.
May need to wait if
- Active urinary infection should be treated first.
- A poorly functioning kidney may be better managed another way, as the specialist will explain.
- Mild, non-progressive swelling without symptoms may simply be observed.
- Uncontrolled medical conditions need optimisation.
How to prepare
- Bring your ultrasound, CT scan and renogram (MAG3 or DTPA) reports that show the obstruction and kidney function.
- Have a urine culture and treat any infection beforehand.
- Tell your doctor about blood thinners and other medicines; changes only on the doctor's instruction.
- Follow fasting instructions and plan an early stent-removal visit before travelling long distances.
- Arrange help at home for the first week and avoid heavy lifting for a few weeks.
Questions to ask your surgeon
- Do I definitely need surgery, and how do you know the blockage is causing my symptoms?
- Which approach do you recommend, laparoscopic, robotic or open?
- How many pyeloplasties do you do, and what is the success rate in your hands?
- Will a stent be placed, and how will it be removed?
- How soon will I feel better, and when will you test kidney drainage again?
- Who can repeat the renogram or ultrasound at home?
Red flags when comparing hospitals
- Surgery recommended without a renogram or clear evidence of obstruction.
- Endopyelotomy is offered as a first choice without discussing its lower success rates.
- Surgeon rarely performs reconstructive kidney surgery.
- No stent removal or follow-up scan plan.
