About Multifocal IOL Implantation

A multifocal IOL is an artificial lens with several focusing zones, giving clear distance, intermediate and near vision. It is for people having cataract or lens-exchange surgery who want to depend less on glasses.

The lens is implanted during cataract or refractive lens exchange surgery, a day case under local or topical anaesthesia with light sedation. A 2 to 3 mm corneal incision is made, the natural lens is broken up and removed, and the folded multifocal lens is placed in the capsule. Bifocal and trifocal designs are the main options.

Usually a good fit if

  • Cataract or presbyopia patients wanting good near, intermediate and distance vision.
  • Healthy macula, optic nerve and cornea.
  • Willing to accept halos and glare as the trade-off for independence from glasses.
  • Regular, low corneal astigmatism, or addressed with a toric version.

May need to wait if

  • Macular degeneration, glaucoma or other conditions reducing contrast vision.
  • Significant night driving or visually demanding night work.
  • Irregular cornea or high astigmatism not corrected.
  • Expectation of perfect, glare-free vision.
How to prepare
  1. Complete biometry, corneal topography and macular OCT; bring details of any previous laser eye surgery.
  2. Discuss night driving, reading and computer needs so the lens fits your lifestyle.
  3. Treat dry eye and eyelid inflammation before measurement.
  4. Review medicines with your doctor; do not stop any without instruction.
  5. Arrange transport and early reviews.
Questions to ask your surgeon
  1. Is a multifocal better than an EDOF or monofocal lens for me?
  2. What will my night vision be like?
  3. What happens if I cannot adapt to the lens?
  4. Will I need glasses for any tasks?
  5. Is a toric multifocal needed?
  6. Who can provide aftercare at home?
Red flags when comparing hospitals
  • No explanation of halos, glare or reduced contrast.
  • No macular scan or corneal mapping.
  • Guaranteed independence from glasses.
  • No lens model record.
