About Mini-PCNL

Mini-PCNL removes kidney stones through a narrow track in the back, using a smaller sheath than standard PCNL. It is aimed at medium to large stones, with a lower risk of bleeding.

Under general anaesthesia, the kidney is punctured under X-ray or ultrasound guidance and the track is widened to a smaller size than in standard surgery. The surgeon passes a telescope along it and breaks the stone with a laser or ultrasonic probe, removing the fragments. A tube in the kidney or a ureteric stent may be left for a day or more.

Usually a good fit if

  • Kidney stones too large for shock wave treatment, often 1.5 to 3 cm.
  • Stones that have failed other treatment or are in lower kidney positions.
  • A patient whose kidney anatomy allows safe puncture.
  • Normal clotting and no active infection.

May need to wait if

  • Active urinary infection must be treated first.
  • Uncorrected bleeding disorders or blood thinners that cannot be paused need a plan.
  • Very large or staghorn stones may require standard PCNL or staged treatment.
  • Severe spine or lung conditions may limit positioning.
How to prepare
  1. Bring your CT scan and recent urine culture; infection must be treated beforehand.
  2. Tell your doctor about blood thinners and aspirin; changes only on the doctor's instruction.
  3. Expect blood tests, clotting studies and an anaesthetic review.
  4. Follow fasting instructions and arrange help for the first days after discharge.
  5. Ask about a nephrostomy tube or stent so you can plan for removal before travel.
Questions to ask your surgeon
  1. Is mini-PCNL the right size of tract for my stone, or would standard PCNL or RIRS be better?
  2. How many mini-PCNLs do you perform and what is the stone-free rate?
  3. Will I have a nephrostomy tube, and for how long?
  4. What are the risks of bleeding, infection and injury to nearby organs?
  5. How long will I stay in hospital?
  6. Who can arrange stone-free scans and prevention advice at home?
Red flags when comparing hospitals
  • Offered without a CT scan to plan the puncture.
  • No discussion of alternatives like RIRS or standard PCNL.
  • Surgeon does not have blood transfusion or ICU backup for bleeding.
  • No plan for the nephrostomy tube, stent or follow-up scan.
