About Leadless Pacemaker (Micra)

The Micra leadless pacemaker is a tiny self-contained device placed directly inside the heart, with no leads or chest pocket. It treats slow heart rhythms, mainly in people who need single-chamber pacing.

Under local anaesthesia with sedation, a catheter is passed through a vein in the groin to the right ventricle, where the capsule-sized pacemaker is anchored to the heart wall. There is no incision in the chest and no visible scar. The procedure is typically shorter than a conventional pacemaker implant, and the device is checked before the catheter is removed.

Usually a good fit if

  • Slow heart rhythm needing single-chamber ventricular pacing, such as permanent atrial fibrillation with slow rates.
  • Higher risk from conventional pacemaker pockets or leads, such as infection risk, dialysis or poor veins.
  • People who prefer no visible device under the skin.
  • Suitable heart size and vein anatomy.

May need to wait if

  • Need for atrial or dual-chamber pacing, which standard Micra cannot provide, although newer versions exist.
  • Active infection or blood-clotting problems.
  • Severe right heart enlargement or blocked femoral veins.
  • Mechanical tricuspid valve.
How to prepare
  1. Have an ECG, rhythm monitoring and echocardiogram, plus blood tests including clotting and kidney function.
  2. Review blood thinners and other medicines with your doctor, who will tell you what to adjust before the procedure.
  3. Tell the team about vein blockage or filters in the leg veins, or previous groin procedures.
  4. Fast for the hours the team specifies.
  5. Plan a device identification card and arrange follow-up near home.
Questions to ask your surgeon
  1. Is a single-chamber leadless device suitable for my rhythm?
  2. How many leadless pacemakers has your team implanted?
  3. What happens when the battery runs low?
  4. What is the risk of heart perforation, and how would you manage it?
  5. Can my home clinic check this device?
  6. Would a conventional pacemaker serve me better?
Red flags when comparing hospitals
  • Leadless device offered when you need dual-chamber pacing without explanation.
  • Team has little experience and no cardiac surgical backup.
  • No device-clinic follow-up arranged.
  • No explanation of battery replacement approach.
  • Claim that leadless pacemakers never have complications.
