About Splenectomy (Laparoscopic)

Laparoscopic splenectomy removes the spleen through small cuts using a camera. It is used for immune thrombocytopenia not controlled by medicines, some blood disorders and certain cysts or tumours.

The operation is done under general anaesthesia and takes about two hours. You lie on your side, and the surgeon makes three or four small cuts in the left upper abdomen. The spleen is freed, its artery and vein are divided with clips or a stapler, and it is placed in a bag and removed in pieces or whole through a small incision. A very large spleen may need open surgery.

Usually a good fit if

  • Immune thrombocytopenia not responding to medicines.
  • Haemolytic anaemia or hereditary spherocytosis.
  • Splenic cysts, tumours or abscesses.
  • Massive splenomegaly in selected cases.

May need to wait if

  • Very large spleen that may need hand-assisted or open surgery.
  • Platelet counts not yet optimised, which the team must manage.
  • Severe heart or lung disease unfit for anaesthesia.
  • Not able to have vaccinations or take infection precautions.
How to prepare
  1. Blood tests and, for immune thrombocytopenia, evidence that steroids or other treatments have failed.
  2. Vaccination against pneumococcus, meningococcus and Haemophilus influenzae, ideally two weeks or more before surgery.
  3. CT or ultrasound to measure spleen size and look for accessory spleens.
  4. Review steroids, blood thinners and other medicines with your doctor; do not change them yourself.
  5. Fast and attend anaesthetic assessment.
Questions to ask your surgeon
  1. Why is splenectomy the right choice for me now?
  2. Will I need platelet transfusion?
  3. Which vaccines and preventive antibiotics do I need?
  4. What are the chances of bleeding and of the condition returning?
  5. Will accessory spleens be searched for?
  6. What should I do if I have a fever after discharge?
Red flags when comparing hospitals
  • No vaccination plan.
  • No haematology review.
  • Surgeon with little splenic experience.
  • No medical alert card.
  • No fever precautions given.

Yes, but you have a lifelong higher risk of serious infection, so vaccines, fever precautions and sometimes antibiotics are important.

Will my platelets improve?

Many people with immune thrombocytopenia respond, but not all, and some relapse.

Do I need vaccines?

Yes. They are best given before surgery, with boosters afterwards.

Why is a medical alert card useful?
